目的：探讨 CA125测值在恶性和结核性胸腔积液鉴别诊断中的价值。方法选取2011年10月—2013年12月在该科住院确诊的胸腔积液患者59例，其中恶性胸腔积液组30例，结核性胸腔积液组29例，对两组胸腔积液及血清中 CA125初次测值进行比较。结果恶性组和结核组中胸腔积液 CA125测值比较差异具有统计学意义（P ＝0．018），鉴别诊断有一定准确性（AUC ＝0．739），两组胸腔积液和血清 CA125比值比较差异具有统计学意义（P ＝0．002），鉴别诊断有一定准确性（AUC ＝0．793），两组血清 CA125测值比较差异不具有统计学意义（P ＝0．139），鉴别诊断有较低准确性（AUC ＝0．575）。结论胸腔积液中的 CA125、胸腔积液和血清 CA125的比值对于恶性和结核性胸腔积液的鉴别有一定的价值，血清中的 CA125对于恶性和结核性胸腔积液的鉴别意义不大。

목적：탐토 CA125측치재악성화결핵성흉강적액감별진단중적개치。방법선취2011년10월—2013년12월재해과주원학진적흉강적액환자59례，기중악성흉강적액조30례，결핵성흉강적액조29례，대량조흉강적액급혈청중 CA125초차측치진행비교。결과악성조화결핵조중흉강적액 CA125측치비교차이구유통계학의의（P ＝0．018），감별진단유일정준학성（AUC ＝0．739），량조흉강적액화혈청 CA125비치비교차이구유통계학의의（P ＝0．002），감별진단유일정준학성（AUC ＝0．793），량조혈청 CA125측치비교차이불구유통계학의의（P ＝0．139），감별진단유교저준학성（AUC ＝0．575）。결론흉강적액중적 CA125、흉강적액화혈청 CA125적비치대우악성화결핵성흉강적액적감별유일정적개치，혈청중적 CA125대우악성화결핵성흉강적액적감별의의불대。
Objective To explore the function of CA125 measurements in differentiating the malignant and tuberculous pleural effu-sion.Methods We selected 59 inpatients with pleural effusion admitted to hospital from October 2011 to December 2013 and assigned them into two groups,the one including 30 cases with malignant pleural effusion and the other including 29 cases of tuberculous pleural effusion.And the CA125 initial value of pleural effusion and serum measured in these two groups were compared.Results The differ-ences of CA125 measurements in the pleural effusion between the two groups were statistically significant (P =0.018),and the differ-ential diagnosis was accurate (AUC =0.739).The CA125 ratio of the pleural effusion and serum between the two groups had statistical significance (P =0.002),and the differential diagnosis had certain accuracy (AUC =0.793).The differences of CA125 measure-ments in the serum between the two groups had no statistical significance (P =0.139),and the differential diagnosis had low accuracy (AUC =0.575).Conclusions Pleural effusion CA125,the CA125 ratio of pleural effusion and serum have significance in differenti-ating the malignant and tuberculous pleural effusion;however,as for the serum CA125,there is no significance.
