目的:探讨微型钛板塑形内固定整复术在眼眶骨折中的应用及疗效。方法选取2009年2月-2014年3月期间该院接收治疗的眼眶骨折患者共103例为研究对象,根据患者采取的内固定方式将103例患者分为观察组和对照组。其中55例患者采取微型钛板塑形内固定治疗为观察组,48例患者采取羟基磷灰石复合人工骨板内固定治疗为对照组。对比两组患者的愈合情况。结果本次微型钛板塑形内固定整复术中,一次性整复微型钛板固定成功27例,粉碎性眼眶骨折患者进行II期整复微型钛板固定38例,所有患者均成功实施整复固定手术,手术后未出现排斥反应或脱落等并发症。随访3个月,经CT扫描检查,55例复合眼眶骨折,骨性眶壁解剖复位良好,整复材料无不良反应,无脱出、移位及感染。眼球内陷、移位恢复50例,占90.1%。52例中全部生活范围复视消失,3例(5.4%)残留周边复视。对照组中整复材料3例出现不良反应,2例脱出、1例移位并发感染,不良反应发生率为6.2%。眼球内陷、移位恢复36例,占65.5%。观察组患者的恢复率为90.1%,不良反应率为0.0%。对照组患者的恢复率1为65.5%,不良反应率2为6.2%。两组患者在恢复率、不良反应发生率上对比差异有统计学意义(χ21=6.384,P<0.05;χ22=5.937,P<0.05)。结论在眼眶骨折的复位固定处理过程中,采用微型钛板塑形内固定整复术可以很好的对眼眶受损软组织进行整复,该方法操作简单,对患者影响较小,是整复眼眶和颜面部骨折理想材料之一,有利于促进患者眼球运动功能的恢复。

목적:탐토미형태판소형내고정정복술재안광골절중적응용급료효。방법선취2009년2월-2014년3월기간해원접수치료적안광골절환자공103례위연구대상,근거환자채취적내고정방식장103례환자분위관찰조화대조조。기중55례환자채취미형태판소형내고정치료위관찰조,48례환자채취간기린회석복합인공골판내고정치료위대조조。대비량조환자적유합정황。결과본차미형태판소형내고정정복술중,일차성정복미형태판고정성공27례,분쇄성안광골절환자진행II기정복미형태판고정38례,소유환자균성공실시정복고정수술,수술후미출현배척반응혹탈락등병발증。수방3개월,경CT소묘검사,55례복합안광골절,골성광벽해부복위량호,정복재료무불량반응,무탈출、이위급감염。안구내함、이위회복50례,점90.1%。52례중전부생활범위복시소실,3례(5.4%)잔류주변복시。대조조중정복재료3례출현불량반응,2례탈출、1례이위병발감염,불량반응발생솔위6.2%。안구내함、이위회복36례,점65.5%。관찰조환자적회복솔위90.1%,불량반응솔위0.0%。대조조환자적회복솔1위65.5%,불량반응솔2위6.2%。량조환자재회복솔、불량반응발생솔상대비차이유통계학의의(χ21=6.384,P<0.05;χ22=5.937,P<0.05)。결론재안광골절적복위고정처리과정중,채용미형태판소형내고정정복술가이흔호적대안광수손연조직진행정복,해방법조작간단,대환자영향교소,시정복안광화안면부골절이상재료지일,유리우촉진환자안구운동공능적회복。
Objective To investigate the application and efficacy of moulding internal fixation plasty with titanium mini-plate in the treatment of orbital fracture. Methods 103 patients with orbital fracture admitted to our hospital between February 2009 and March 2014 were selected as the research object and divided into observation group and control group according to the method of internal fixation. 55 patients in the observation group received moulding internal fixation plasty with titanium mini-plate, while 48 patients in the control groups received internal fixation with hydroxyapatite artificial bone. Healing of the two groups was com-pared.Results The miniature titanium shaping fixed pasty, a one-time success of the entire complex micro-plate fixation in 27 cases of orbital fractures were com minuted entire complex miniature titanium plate 38 cases of stage II, all patients were success-fully implemented the entire complex fixation reaction or loss and other complications after surgery does not appear exclusive. Fol-low-up in March, by CT scan, 55 cases of complex orbital fractures, orbital bone anatomically well, the entire complex materials no adverse reaction, no prolapse, displacement and infection. Ophthalmologist, shift recover 50 cases, accounting for 90.1%. 52 cases in all living areas diploma disappeared, three cases (5.4%) residues surrounding diploma. The control group, the entire com-plex material adverse reactions occur three cases, two cases of prolapse, one case shift concurrent infections, adverse reactions was 6.2%. Ophthalmologist, shift recover 36 cases, 65.5%. Observation group were 90.1%recovery rate of adverse reactions was 0.0%. The recovery rate in the control group 65.5%of patients 1, 2 rate of adverse reactions was 6.2%. Comparing two groups of patients in the recovery rate, incidence of adverse reactions was significantly (χ21=6.384,P<0.05;χ22=5.937,P<0.05).Conclusion In the pro-cess of orbital fracture fixation , the use of moulding internal fixation plasty with titanium mini-plate can be good for orbital soft tissue damage restoration. The method is simple and has less impact on the patient. It is the ideal material for orbital and facial fractures and can help to promote recovery of motor function.
