目的：总结2014年6月至9月我院住院医嘱问题用药的构成，并对其中的典型案例进行分析。方法：收集并总结药师干预的医嘱，归纳用药不合理类型，分析其中原因。结果：在所有不合理医嘱中，药品的溶媒选择错误和用法用量错误的医嘱占所有不合理医嘱的比例较高，分别为46.89%和41.30%。结论：药品的溶媒选择和用法用量非常容易出现错误，药品的相互作用、重复用药和配伍禁忌问题更不容忽视，值得临床注意。

Objective:The article aims to summarize the common problems in the hospital ward and the distribution of the problems,and analyze the typical cases.Methods: The manuscript collected and summarized the inpatients medical orders, summarized the type of problems, analyzed the reasons.Results: In all the problematically medical orders, the selection of the drug menstruum and the usage error of the medical orders occupied a higher proportion, respectively 46.89% and 41.30%. Conclusions: It is easy to have errors for the selection of the drug menstruum and usage of drugs. Drug interactions, repeated drug use and incompatibility problems cannot be ignored. It is worthy of attention for the clinical doctors.
