目的：探讨结核T细胞斑点检测（T-SPOT．TB）和皮肤结核菌素试验（TST）在结核诊断中的临床意义。方法选取甘孜藏族自治州人民医院2012年4月至2014年1月收治的99例结核病疑似患者为研究对象，分别对患者采取T-SPOT．TB试验和TST进行检查，比较两种检测方法的诊断效能以探讨其临床意义。结果 T-SPOT．TB 的特异度为17.2％，灵敏度为80.8％，阳性预测值为81.2％，阴性预测值为89.4％，准确度为84.2％。 TST的特异度为79.3％，灵敏度为39.0％，阳性预测值为72.7％，阴性预测值为79.3％，准确度为67.4％。结论结核病应用T-SPOT．TB诊断的准确度较TST试验更高，对结核病的诊断具有重要意义。

Objective To investigate the clinical value between tuberculosis T cell spot detection ( T-SPOT.TB) and tuberculin skin test( TST) in diagnosis of tuberculosis.Methods A total of 99 patients with suspected tuberculosis admitted in Ganzi People′s Hospital from Apr.2012 to Feb.2014 were chosen as the experimental objects.All the patients received T-SPOT.TB test and TST test,the sensitivity,specificity, and diagnostic capacity of the two methods were compared, and their clinical significance was explored. Results Thespecificity of T-SPOT.TB was 17.2%,the sensitivity was 80.8%,the positive predictive value was 81.2%,the negative predictive value was 89.4%,and the accuracy was 84.2%.While the specificity of TST was 79.3%,the sensitivity was 39.0%,the positive predictive value was 72.7%,the negative predic-tive value was 79.3%,the accuracy was 67.4%.Conclusion The diagnostic accuracy of T-SPOT.TB is higher than that of TST in the diagnosis of tuberculosis,therefore,T-SPOT.TB has greater significance the diagnosis of tuberculosis.
