献血者丙氨酸转氨酶（ ALT）检测不合格曾是血液报废的主要原因，一度困扰着国内血液机构。随着酶联免疫吸附试验和核酸扩增检测技术的进步，ALT值最早引入献血者筛查的目的已经发生了转变。相比于国际上许多国家的做法，国内输血界对现行国家标准产生了质疑，是否取消ALT的筛查或改变ALT筛查限值的讨论曾成为国内血液行业热议的焦点，同时许多地区对当地献血者ALT参考值开展了调查。多项举措降低了ALT不合格的血液报废，使该难题的解决看到了曙光。

Unqualified alanine aminotransferase(ALT) levels in blood donors has been the main reason for the blood discarding,which has been a big problem in domestic blood institutions.With the development of enzyme-linked immunosorbent assay and polymerase chain reaction detection technologies,the purpose of the ALT value first introduced in the screening of blood donors has been changed.Compared with the international practice in many countries,existing domestic blood transfusion standard has been questioned.The discussion about whether to cancel ALT screening or change the limit value of ALT has become the focus in domestic blood transfusion.At the same time,investigation on ALT reference values of local blood donors has been car-ried out in many regions.A number of measures have been applied to reduce the blood discarding rate in blood donors with unqualified ALT,which have made the resolution of the problem see the dawn.
