目的:探讨以案例为基础的教学(Case-Based Learning,CBL)与以问题为基础的教学(Problem-Based Learning, PBL)双轨式结合,在胃肠外科住院医师规范化培训中的应用效果。方法:选取在胃肠外科轮转的37名规范化培训学员,对其进行CBL+ PBL双轨教学模式培训,通过出科考核及问卷调查两种方式,采用自身前、后对照方法,评价学员的教学效果。结果:学员出科考试成绩CBL+PBL双轨教学模式明显高于传统教学模式(P<0.05)。调查问卷结果提示CBL+PBL双轨教学模式明显提高学员的自学能力、临床思维能力、查阅资料能力及语言表达能力(P<0.05),差异有统计学意义。结论:CBL+PBL双轨教学模式有利于提高胃肠外科轮转的住院医师综合能力,但还需要不断探索和实践。

Objective To explore the application effect on the combination of Case Based Learning(CBL) and Problem Based Learning(PBL) in resident standardization training of gastrointestinal surgery. Methods 37 residents of standardization training in gastrointestinal surgery were selected, and they were trained with CBL+PBL dual-track teaching mode. We evaluated the teaching effects before and after the CBL+PBL teaching mode by given examinations and questionnaire surveys. Results Students in CBL+PBL dual-track teaching mode shown better performance compared with traditional teaching mode (P<0.05). Survey results indicated that students' self-study ability and clinical thinking ability, informative ability and language expression ability (P<0.05). were obviously increased by CBL+PBL teaching mode. Conclusion CBL+PBL dual-track teaching model can help improve the comprehensive abilities of residents of standardization training in gastrointestinal surgery, but it also needs to explore and practice continuously.
