目的：探讨腰麻伴硬膜外盐水扩充治疗剖宫产术后下肢运动阻滞的临床效果。方法选取2009年10月~2012年10月本院收治的98例择期剖宫产产妇，随机分为观察组和对照组，每组49例。所有患者术前肌注阿托品0.5 mg后，观察组给予腰麻复合硬膜外盐水扩充，对照组采用布比卡因腰硬联合麻醉。观察两组产妇生命体征变化情况，并比较两组产妇感觉、运动阻滞及恢复情况。结果两组麻醉前后收缩压(SBP)、舒张压(DBP)和心率(HR)等生命体征均无显著性变化(P>0.05)；观察组感觉阻滞持续时间为(215.3±42.7) min，较对照组的(246.4±39.2)min明显缩短，观察组运动阻滞持续时间为(204.5±32.4)min，较对照组的(228.4±34.5)min短差异均有统计学意义(P<0.05)。结论小剂量腰麻伴硬膜外盐水扩充可满足临床剖宫产手术要求，且显著缩短产妇术后下肢感觉、运动阻滞时间。

목적：탐토요마반경막외염수확충치료부궁산술후하지운동조체적림상효과。방법선취2009년10월~2012년10월본원수치적98례택기부궁산산부，수궤분위관찰조화대조조，매조49례。소유환자술전기주아탁품0.5 mg후，관찰조급여요마복합경막외염수확충，대조조채용포비잡인요경연합마취。관찰량조산부생명체정변화정황，병비교량조산부감각、운동조체급회복정황。결과량조마취전후수축압(SBP)、서장압(DBP)화심솔(HR)등생명체정균무현저성변화(P>0.05)；관찰조감각조체지속시간위(215.3±42.7) min，교대조조적(246.4±39.2)min명현축단，관찰조운동조체지속시간위(204.5±32.4)min，교대조조적(228.4±34.5)min단차이균유통계학의의(P<0.05)。결론소제량요마반경막외염수확충가만족림상부궁산수술요구，차현저축단산부술후하지감각、운동조체시간。
Objective To explore the clinical effect of lumbar anesthesia combining with epidural saline expansion in the treatment of motor block of lower limbs after caesarean section. Methods 98 puerperae with cesarean delivery ad-mitted into our hospital from October 2009 to October 2012 were selected,and they were randomly divided into obser-vation group and control group,with 49 cases in each group.Before surgery,they were all intramuscularly injected with atropine 0.5 mg.In the observation group,lumbar anesthesia combining with epidural saline expansion was adopted. While in the control group,bupivicaine for combined spinal and epidural anesthesiawas applied.The changes of vital signs in the two groups was observed,and block and recovery of puerperal motor and sensory in two groups was com-pared. Results There was no significant change in systolic blood pressure(SBP),diastolic blood pressure(DBP),and heart rate(HR) in the two groups(P>0.05).In the observation group,sensory block duration was (215.3±42.7) min,which was short-er than that of control group [(246.4±39.2) min].Motor block duration in the observation group was (204.5±32.4) min, which was shorter than that of control group[(228.4±34.5) min],the difference was significant(P<0.05). Conclusion Low-dose spinal anesthesia with epidural saline expansion can meet the requirements of cesarean section in clinic and sig-nificantly shorten puerperal sensoryand motor block time of lower limbs after surgery.
