目的：观察辛伐他汀联合曲美他嗪对2型糖尿病伴冠心病患者高敏C反应蛋白(hs-CRP)、血糖、胰岛素抵抗及血脂的影响。方法选择2013年11月~2014年11月本院收治的2型糖尿病伴冠心病患者60例，所有患者糖化血红蛋白均<7%，均未使用胰岛素治疗，给予辛伐他汀(20 mg，1次/晚)联合盐酸曲美他嗪(20 mg，3次/d)治疗12周，观察治疗前后hs-CRP、血糖、胰岛素及血脂的变化，并计算胰岛素敏感指数(ISI)。结果治疗后hs-CRP较治疗前降低(P<0.05)，总胆固醇、三酰甘油及低密度脂蛋白胆固醇较治疗前降低(P<0.05)，空腹胰岛素和餐后2 h胰岛素水平较治疗前降低(P<0.05)，治疗后胰岛素敏感性增大(P<0.05)，治疗前后血糖水平无明显变化(P>0.05)。结论辛伐他汀联合曲美他嗪可降低2型糖尿病合并冠心病患者hs-CRP及血脂水平，同时可增加胰岛素敏感性。

목적：관찰신벌타정연합곡미타진대2형당뇨병반관심병환자고민C반응단백(hs-CRP)、혈당、이도소저항급혈지적영향。방법선택2013년11월~2014년11월본원수치적2형당뇨병반관심병환자60례，소유환자당화혈홍단백균<7%，균미사용이도소치료，급여신벌타정(20 mg，1차/만)연합염산곡미타진(20 mg，3차/d)치료12주，관찰치료전후hs-CRP、혈당、이도소급혈지적변화，병계산이도소민감지수(ISI)。결과치료후hs-CRP교치료전강저(P<0.05)，총담고순、삼선감유급저밀도지단백담고순교치료전강저(P<0.05)，공복이도소화찬후2 h이도소수평교치료전강저(P<0.05)，치료후이도소민감성증대(P<0.05)，치료전후혈당수평무명현변화(P>0.05)。결론신벌타정연합곡미타진가강저2형당뇨병합병관심병환자hs-CRP급혈지수평，동시가증가이도소민감성。
Objective To observe influences of simvastatin combined with trimetazidine on hypersensitive c-reactive protein (hs-CRP), blood glucose, insulin resistance and blood lipids in type 2 diabetes mellitus patients combined with coronary heart disease (CHD). Methods Sixty patients with type 2 diabetes mellitus and CHD whose glycosylated hemoglobin was less than 7%in all participants who were not applied with insulin for treatment were added with 20 mg simvastatin once per night and 20 mg trimetazidine hydrochloride three times per day for 12 weeks treatment.The changes of hs-CRP,blood glucose,insulin and blood lipids before and after treatment was observed and insulin sensitivi-ty index (ISI) was calculated respectively. Results After treatment,hs-CRP was reduced compared with prior treatment (P<0.05).The total cholesterol,triglycerides and low-density lipoprotein cholesterol was lower than that before treatment espectively(P<0.05).Levels of fasting insulin and 2 hours postprandial insulin was decreased in comparison with that be-fore treatment respectively (P<0.05).Insulin sensitivity was increased after treatment (P<0.05).There was no significant changes in blood glucose levels before and after treatment (P>0.05). Conclusion Simvastatin combined with trimetazi-dine can decrease the levels of hs-CRP and lipids,but increase insulin sensitivity in patients with type 2 diabetes mel-litus and CHD.
