目的：观察并分析应用PVP和PKP在治疗骨质疏松性椎体压缩骨折（OVCF）的临床效果。方法选取2010年7月～2014年12月在我院接受治疗的100例OVCF患者作为研究对象，其中应用PVP、PKP进行治疗的患者各50例。观察两组患者术后Cobb角矫正情况、椎体前缘高度、椎体后缘高度、术后疼痛情况及并发症发生情况。结果PKP组的矫正效果优于PVP组，其中PKP组的术后Cobb角矫正情况、椎体前缘高度、椎体后缘高度均优于PVP组，P＜0.05，差异具有统计学意义；两组患者各个时期的疼痛评分没有差异，P＞0.05，差异不具有统计学意义；骨水泥向椎前及椎旁少量渗漏15例，其中PVP 11例，较PKP组4例为高，均未引起症状，未行特殊处理，均未发生脊髓、神经根损伤及过敏、肺栓塞等严重并发症。结论PVP和PKP治疗骨质疏松性椎体压缩骨折均具有微创、疗效满意且并发症少等优点，是治疗老年OVCF的有效方法；PKP在恢复椎体高度、矫正后凸畸形、减少骨水渗漏方面更具优势。
목적：관찰병분석응용PVP화PKP재치료골질소송성추체압축골절（OVCF）적림상효과。방법선취2010년7월～2014년12월재아원접수치료적100례OVCF환자작위연구대상，기중응용PVP、PKP진행치료적환자각50례。관찰량조환자술후Cobb각교정정황、추체전연고도、추체후연고도、술후동통정황급병발증발생정황。결과PKP조적교정효과우우PVP조，기중PKP조적술후Cobb각교정정황、추체전연고도、추체후연고도균우우PVP조，P＜0.05，차이구유통계학의의；량조환자각개시기적동통평분몰유차이，P＞0.05，차이불구유통계학의의；골수니향추전급추방소량삼루15례，기중PVP 11례，교PKP조4례위고，균미인기증상，미행특수처리，균미발생척수、신경근손상급과민、폐전새등엄중병발증。결론PVP화PKP치료골질소송성추체압축골절균구유미창、료효만의차병발증소등우점，시치료노년OVCF적유효방법；PKP재회복추체고도、교정후철기형、감소골수삼루방면경구우세。
Objective To observe the application and analysis clinical effect on PVP and PKP in the treatment of osteoporotic vertebral compression fractures (OVCF). Methods Selected 100 cases with OVCF from 2010 July to 2014 December in our hospital as the research object, the application of PVP, PKP for the treatment of patients with 50 cases in each. Observed two groups of patients with postoperative Cobb angle, anterior vertebral height correction, posterior margin of the vertebral body height, postoperative pain and complications. Results In group PKP, the correction effect was better than that in PVP group, the PKP group, the postoperative Cobb angle, anterior vertebral height correction, posterior margin of the vertebral body height was higher than that the group PVP, P<0.05, had difference statistically signiifcance, there were no difference between the two groups in each period of pain score, P>0.05, had no difference statistically significance, bone cement into the vertebral and paravertebral a small amount of leakage in 15 cases, included PVP 11 cases, PKP group of 4 cases was obviously high, did not cause obvious symptoms, without special treatment. There were no serious complications of spinal cord, nerve root injury and allergies, pulmonary embolism. Conclusion PVP and PKP in the treatment of osteoporotic vertebral compression fractures with advantages of minimally invasive, curative effect and less complications, is an effective method for the treatment of elderly OVCF, PKP has more advantages on the recovery of vertebral height, kyphosis correction, reduce the water leakage of bone.
