目的:探讨改良体位在早产儿PICC导管异位于对侧锁骨下和腋静脉的处理效果。方法:选取2013年6月-2014年12月行PICC置管导管尖端异位于对侧锁骨下(A组)和腋静脉(B组)的早产儿,分别采用不同的改良体位方法对异位的导管进行调整,监测调整后效果。结果:异位于对侧锁骨下静脉试验A组一次纠正成功率高于对照A组,差异有统计学意义(P<0.05),异位于腋静脉试验B组一次纠正成功率与对照B组比较差异无统计学意义(P>0.05)。异位于对侧锁骨下和腋静脉试验组导管留置时间与对照组比较差异均有统计学意义(P<0.05),而静脉炎发生率比较差异均无统计学意义(P>0.05)。结论:采取改良体位对PICC导管异位于对侧锁骨下和腋静脉进行正位,操作简单、方便,可以减少输液外渗,减少患者的痛苦,减轻家庭负担。

목적:탐토개량체위재조산인PICC도관이위우대측쇄골하화액정맥적처리효과。방법:선취2013년6월-2014년12월행PICC치관도관첨단이위우대측쇄골하(A조)화액정맥(B조)적조산인,분별채용불동적개량체위방법대이위적도관진행조정,감측조정후효과。결과:이위우대측쇄골하정맥시험A조일차규정성공솔고우대조A조,차이유통계학의의(P<0.05),이위우액정맥시험B조일차규정성공솔여대조B조비교차이무통계학의의(P>0.05)。이위우대측쇄골하화액정맥시험조도관류치시간여대조조비교차이균유통계학의의(P<0.05),이정맥염발생솔비교차이균무통계학의의(P>0.05)。결론:채취개량체위대PICC도관이위우대측쇄골하화액정맥진행정위,조작간단、방편,가이감소수액외삼,감소환자적통고,감경가정부담。
Objective:To study the treatment effect of improved posture,located at the side of the subclavian and axillary vein catheter in premature infant PICC catheter.Method:Premature infants with PICC catheter from June 2013 to December 2014 were selected,whose tips were on the side of the subclavian and axillary vein. Different methods were used to improved posture of ectopic catheter adjustment, monitoring after adjustment effect.Result:The contralateral subclavian vein experimental A group once correct success rate was higher than the control A group,with statistically significant differences(P<0.05),the axillary vein once correct success rate in the experimental B group was higher than the control B group,no statistical significance (P>0.05).Catheteriztion time in the contralateral subclavian and axillary vein catheter of the experimental group had obvious differences statistically significant(P<0.05).Two groups in the incidence of phlehitis had no statistical significance difference (P>0.05).Conclusion:Adopting improved position in PICC catheter is carried out in the side of the subclavian and axillary vein,it is a simple operation,conveniently reduce the transfusion exosmosis,the suffering painess of patients and the burdens of families.
