目的:探究分析多层螺旋CT三维重建技术在踝关节骨折诊断和术前评估中的应用价值。方法:分别采用多层螺旋CT三维重建技术及X线DR对踝关节骨折患者进行术前检查并进行分型,分别在术后进行复查以观察患者的复位和内固定置入质量。结果:术前DR分型与术中结果比较中,PAB、SAB、PER及SER行的Kappa值一致性良好。CT三维重建与术中结果的一致性、特异度及灵敏度都优于DR。术后CT三维重建对PAB、PER及SER的复位质量判断的准确率明显优于DR。结论:多层螺旋CT三维重建技术在踝关节骨折诊断中术前分型、术后复位治疗和内固定置入质量的判断方面均有很高的准确性,并且可靠性高,具有较高的临床运用价值。

Objective:To explore and analyze the value of multilayer spiral CT 3D reconstruction technique in the application of ankle fracture diagnosis and preoperative evaluation.Method:Multilayer spiral CT three-dimensional reconstruction technique and X-ray DR were given to the patients with ankle fractures for preoperative examination,classification and reexamination after surgery.Result:Comparing the preoperative DR parting with intraoperative result,the Kappa value in the lines of PAB,SAB,PER and SER was good.Compared with the intraoperative result,the consistency of results,specific and sensitivity of CT three-dimensional reconstruction were superior than those of the DR examination.CT three-dimensional reconstruction was much better than DR in the aspect of reexamination accuracy rate after surgery.Conclusion:Multislice CT three-dimensional reconstruction technique is accurate and reliable in the judgement of ankle fractures patients’ preoperative classification and reexamination after surgery.
