目的：观察扩大切除额极胶质瘤的临床疗效。方法2010年6月至2013年6月我院接诊的100例额极胶质瘤患者，其中50例行扩大切除肿瘤手术治疗（观察组），另50例采用不扩大切除手术治疗（对照组）。比较两组患者术后 KPS评分、中位生存期、复发人数、头痛和癫痫治疗效果与并发症。结果观察组术后头痛、癫痫治疗效果、KPS 评分及中位生存期均优于对照组，复发率明显低于对照组，差异均有有统计学意义（P ＜0．05）。两组术后并发症发生率比较，差异无统计学意义（P ＞0．05）。观察组低病理类型患者的术后 KPS 评分、中位生存期均高于高病理类型患者，复发人数少于高病理类型患者，差异有统计学意义（P ＜0．05）。结论扩大切除额极胶质瘤疗效良好，能显著提高治疗效果和降低复发率，值得临床推广。
목적：관찰확대절제액겁효질류적림상료효。방법2010년6월지2013년6월아원접진적100례액겁효질류환자，기중50례행확대절제종류수술치료（관찰조），령50례채용불확대절제수술치료（대조조）。비교량조환자술후 KPS평분、중위생존기、복발인수、두통화전간치료효과여병발증。결과관찰조술후두통、전간치료효과、KPS 평분급중위생존기균우우대조조，복발솔명현저우대조조，차이균유유통계학의의（P ＜0．05）。량조술후병발증발생솔비교，차이무통계학의의（P ＞0．05）。관찰조저병리류형환자적술후 KPS 평분、중위생존기균고우고병리류형환자，복발인수소우고병리류형환자，차이유통계학의의（P ＜0．05）。결론확대절제액겁효질류료효량호，능현저제고치료효과화강저복발솔，치득림상추엄。
Objective To observe the efficacy of extended resection of gliomas in frontal pole .Methods One hundred pa-tients with gliomas in frontal pole from June 2010 to June 2013 in our hospital were included in the study .They were randomly divided into observation and control groups ,50 in each group.The observation group was treated through extended resection while the control group was treated with non-expanded resection surgery.Clinical data,postoperative KPS,median survival period,number of relapse, treatment effects of headaches and epilepsy and complications were compared between the two groups .Results After treatment,the effect of headache,the epilepsy disappearance,KPS and the median survival time in the observation group were better than that in the control group.Postoperative KPS score in the observation group was higher than that in the control group (P <0.05).The median sur-vival time in the observation group was longer than that in the control group ,and the relapse rate in the observation group was lower than that in the control group (all P <0.05).No statistical significance in postoperative complications ratio was found between the two groups.Postoperative KPS,the median survival time of patients with low histological type was higher than that of patients with the high histological type.The number of relapse in patients with low histological type was less than that in patients with the high histological type (P <0.05).Conclusion Efficacy of extended resection for gliomas in frontal pole was good .It can significantly improve the ther-apeutic effect and reduce the recurrence rate .It is worthy of promotion in clinic.
