目的:探讨碳酸镧治疗慢性肾衰竭并发高磷血症患者的疗效。方法以碳酸镧(lanthanum carbonate)、高磷血症(hyperphosphatemia)、安慰剂(placebo)、碳酸钙(calcium carbonate)、终末期肾病(end-stage renal disease)等为关键字,检索中国期刊全文数据库、中文科技期刊数据库、万方数据知识服务平台、Web of Science、PubMed等数据库,对文献的质量评价后进行Meta分析。结果共纳入10篇,经Meta分析结果显示,对于治疗后血磷水平,WMD=-0.60,95%CI:-0.75~-0.45,碳酸镧与安慰剂在控制血磷水平上差异有统计学意义;对于治疗后血钙水平,WMD=-0.01,95%CI:-0.07~0.05,碳酸镧与安慰剂在控制血钙水平上差异无统计学意义;对于治疗后血PTH水平,WMD=-29.75,95%CI:-39.22~-20.28,说明碳酸镧与安慰剂在控制血PTH水平上差异有统计学意义。对于治疗后血磷水平,WMD=-0.41,95%CI:-0.48~-0.34,碳酸镧与碳酸钙在控制血磷水平上差异有统计学意义;对于治疗后血钙水平,WMD=-0.19,95%CI:-0.25~-0.13,碳酸镧与碳酸钙在控制血钙水平上有统计学意义;对于治疗后血PTH水平,WMD=-174.66,95%CI:-198.86~-150.46,碳酸镧与碳酸钙在控制血 PTH 水平上差异有统计学意义。结论碳酸镧可明显降低慢性肾衰竭并发高磷血症患者的血磷和PTH,对血钙无明显影响,在有效性上优于碳酸钙。

Objective To investigate the efect of lanthanum carbonate treating chronic renal failure with high phosph- orus.Methods Lanthanum carbonate,high phosphorus hyperlipidemia hyperphosphatemia placebo,calcium carbonate, final end-stage renal disease as key words,are retrievaled in China journal ful text database,Chinese science technology periodical database,Wanfang Data of Knowledge service platform,web of science,PubMed database,after the evaluation of the quality of the literature for meta analysis.Results A total of 10 papers, by the meta analysis results showed that,after the treatment,in serum phosphorus level WMD=-0.60,95%CI:-0.75 to-0.45,the difference of lanthanum carbonate comparing with placebo in the control of serum phosphorus level have statistical significance;for blood calcium levels after treatment,WMD=-0.01,95%CI:-0.07 to 0.05,the diference of lanthanum carbonate comparing with placebo in controling serum calcium levels were not statisticaly significant;for PTH levels after treatment,WMD=-29.75,95%CI:-39.22 to-20.28,the difference of lanthanum carbonate comparing with placebo in the control of serum PTH level was statisticaly significant;For the serum phosphorus level after treatment, WMD=-0.41,95%CI:-0.48 to-0.34,the difer- rence of lanthanum c arbonate comparing with calcium are obviously different, after treatment for blood calcium levels, WMD=-0.19,95%CI:-0.25 to-0.13,the difference of lanthanum carbonate comparing with calcium,there were significant differences,for the blood PTH level after treatment,WMD=-174.66,95%CI:-198.86 to-150.46,the diference of lanthanum carbonate comparing with calcium,there were significant differences.Conclusion Lanthanum carbonate can significantly reduce serum phosphorus and PTH,had no significant efect on serum calcium.The efectiveness of Lanthanum carbonate is better than that of calcium carbonate.
