目的:探讨血清脂蛋白a[Lp(a)]、同型半胱氨酸(Hcy)和肱踝脉搏波传导速度(baPWV)与冠心病病变血管的关系。方法选取2012年1月~2013年12月本院确诊的138例冠心病患者作为冠心病组,同时选取62例冠状动脉造影检查(CAG)正常者作为对照组,其中冠心病组又分为单支病变组54例,双支病变组51例,多支病变组33例。采用免疫比浊法测定Lp(a),荧光免疫分析法测定血清Hcy及动脉硬化检测仪检测baPWV。结果冠心病组的血清Lp(a)、HCY、baPWV水平显著高于非冠心病组,差异有统计学意义(P<0.05)。多支病变组的Lp(a)、Hcy、baPWV水平高于单支及双支病变组,差异有统计学意义(P<0.05)。双支病变组的Lp(a)、Hcy、baPWV水平高于单支病变组,差异有统计学意义(P<0.05)。结论血清Lp(a)、Hcy、baPWV水平与冠心病病变血管的病变程度相关,三者结合起来可以用来辅助诊断冠心病及其病变血管的病变程度,从而为临床更有效地诊治冠心病提供可靠的依据。

목적:탐토혈청지단백a[Lp(a)]、동형반광안산(Hcy)화굉과맥박파전도속도(baPWV)여관심병병변혈관적관계。방법선취2012년1월~2013년12월본원학진적138례관심병환자작위관심병조,동시선취62례관상동맥조영검사(CAG)정상자작위대조조,기중관심병조우분위단지병변조54례,쌍지병변조51례,다지병변조33례。채용면역비탁법측정Lp(a),형광면역분석법측정혈청Hcy급동맥경화검측의검측baPWV。결과관심병조적혈청Lp(a)、HCY、baPWV수평현저고우비관심병조,차이유통계학의의(P<0.05)。다지병변조적Lp(a)、Hcy、baPWV수평고우단지급쌍지병변조,차이유통계학의의(P<0.05)。쌍지병변조적Lp(a)、Hcy、baPWV수평고우단지병변조,차이유통계학의의(P<0.05)。결론혈청Lp(a)、Hcy、baPWV수평여관심병병변혈관적병변정도상관,삼자결합기래가이용래보조진단관심병급기병변혈관적병변정도,종이위림상경유효지진치관심병제공가고적의거。
Objective To explore the association of coronary heart disease (CHD) with plasma Lp(a),Hcy and brachial-ankle PWV (baPWV). Methods 138 patients diagnosed CHD in our hospital from January 2012 to December 2013 were selected as the CHD group,62 cases of healthy subjects by CAG were selected as the control group,the CHD group further was divided into single-branch-affected (SBA) group (n=54),two-branch-affected (TBA) group (n=51) and mul-ti-branch-affected (MBA) group (n=33).The level of Plasma Lp(a) was measured by the immunoturbidimetry,the level of Hcy was measured by the fluorescence immunoassay and baPWV was measured using the detector of artery hardening. Results The level of Lp(a),Hcy and baPWV in the CHD group was higher than that in the non-CHD group,with signifi-cant difference(P<0.05).The level of Lp(a),Hcy and baPWV in the MBA group was higher than that in the SBA and the TBA group,with significant difference (P<0.05).The level of Lp (a),Hcy and baPWV in the TBA group was higher than that in the SBA group,with significant difference(P<0.05). Conclusion The level of Lp(a),Hcy and baPWV is correlated with the severity degree of CHD.The combination of three detection can be used to predict CHD and the hazard degree, which can provide reliable diagnosis for more effective prevention and treatment of CHD on clinic.
