目的:探讨妊娠高血压疾病与血管细胞粘附因子的相关性。方法选取自2012年10月至2014年10月收治的48例妊娠高血压疾病的患者(妊高症组),并随机选取正常妊娠妇女30例(正常妊娠组)及正常非妊娠妇女30例(正常非妊娠组)作为两个对照组。采用双抗体夹心酶联免疫吸附法( ELISA)测定各组妇女血清中血管细胞黏附分子-1(VCAM -1)和白细胞介素6(IL -6)的浓度,并采用免疫组织化学染色法检测30例妊高症患者,20例正常妊娠妇女的胎盘组织中 VCAM -1的表达。对比各组妇女血清中 VCAM -1和 IL -6的浓度,以及妊高症组和正常妊娠组胎盘组织中 VCAM -1的表达情况。结果妊高症组血清 VCAM -1浓度、IL -6含量均明显高于正常妊娠组和正常非妊娠组,组间对比差异有显著性( P ﹤0.01)。VCAM -1和 IL -6的浓度水平呈正相关( r =0.62,P ﹤0.01)。VCAM -1在妊高症组胚胎滋养叶细胞的表达明显低于正常妊娠组(23.33% vs.65.00%),差异有显著性(χ2=10.85,P ﹤0.005);妊高症组的毛细血管内皮细胞阳性率高于正常晚期妊娠组(43.33% vs.30.00%),但差异无显著性(χ2=0.95,P ﹥0.05)。结论血清中 VCAM -1的升高参与了妊高症组血管内皮损伤过程,IL -6可诱导 VCAM -1的表达;VCAM -1在妊高症组患者的胚胎滋养细胞呈低表达,表明妊高症患者缺乏血管内皮表型,VCAM -1在妊高症的病理变化中起一定的作用。

목적:탐토임신고혈압질병여혈관세포점부인자적상관성。방법선취자2012년10월지2014년10월수치적48례임신고혈압질병적환자(임고증조),병수궤선취정상임신부녀30례(정상임신조)급정상비임신부녀30례(정상비임신조)작위량개대조조。채용쌍항체협심매련면역흡부법( ELISA)측정각조부녀혈청중혈관세포점부분자-1(VCAM -1)화백세포개소6(IL -6)적농도,병채용면역조직화학염색법검측30례임고증환자,20례정상임신부녀적태반조직중 VCAM -1적표체。대비각조부녀혈청중 VCAM -1화 IL -6적농도,이급임고증조화정상임신조태반조직중 VCAM -1적표체정황。결과임고증조혈청 VCAM -1농도、IL -6함량균명현고우정상임신조화정상비임신조,조간대비차이유현저성( P ﹤0.01)。VCAM -1화 IL -6적농도수평정정상관( r =0.62,P ﹤0.01)。VCAM -1재임고증조배태자양협세포적표체명현저우정상임신조(23.33% vs.65.00%),차이유현저성(χ2=10.85,P ﹤0.005);임고증조적모세혈관내피세포양성솔고우정상만기임신조(43.33% vs.30.00%),단차이무현저성(χ2=0.95,P ﹥0.05)。결론혈청중 VCAM -1적승고삼여료임고증조혈관내피손상과정,IL -6가유도 VCAM -1적표체;VCAM -1재임고증조환자적배태자양세포정저표체,표명임고증환자결핍혈관내피표형,VCAM -1재임고증적병리변화중기일정적작용。
Objective To explore the pathogenesis of vascular cell adhesion analysis of hypertensive disorders in pregnancy factor correla-tion. Methods In our hospital from October 2012 to October 2014,48 cases of patients with hypertensive disorders of pregnancy were treated and included into this study. At the same time,we randomly selected 30 cases of normal pregnant women(normal pregnancy group)and 30 cases of normal non - pregnant women(normal non - pregnant group)as the two control groups,using double - antibody sandwich enzyme - linked immu-nosorbent assay(ELISA)determination of VCAM - 1 concentration of women in each group. The serum IL - 6 was detected by the immunohisto-chemical staining in the 30 cases of PIH patients. The expression of VCAM - 1was detected in 20 cases of normal pregnant women. Comparison se-rum concentrations of VCAM - 1 and IL - 6 in the women,as well as pregnancy - induced hypertension combination normal pregnancy placenta ex-pression of VCAM - 1 were performed. Results The serum levels of VCAM - 1 concentration and serum IL - 6 levels in pregnancy - induced hy-pertension was significantly higher than the normal pregnancy group. There was significant difference between the two groups( P ﹤ 0. 01). VCAM- 1 and IL - 6 levels were positively correlated( r = 0. 62,P ﹤ 0. 01). VCAM - 1 in patients with hypertensive disorders of pregnancy tropho-blast embryonic leaf cells was significantly lower than normal pregnancy(23. 33% vs. 65. 00% ),there was significant difference(χ2 = 10. 85,P ﹤ 0. 005). The positive expression of patients with hypertensive disorders of pregnancy trophoblast embryonic and normal pregnancy did not show significant difference(43. 33% vs. 30. 00% ,χ2 = 0. 95,P ﹥ 0. 05). Conclusion Elevated serum VCAM - 1's participation in the hypertensive disorders of pregnancy vascular endothelial damage process were found. The IL - 6 induces the expression of VCAM - 1. The VCAM - 1 in pa-tients with hypertensive disorders in pregnancy embryonic trophoblast cells showed low expression. Pregnancy - induced hypertension showed a lack of endothelial phenotype,VCAM - 1 plays a role in the pathological changes of pregnancy - induced hypertension.
