目的:探讨沙漠干热环境下中暑大鼠血清炎症因子肿瘤坏死因子(TNF -α)、白细胞介素6(IL -6)、白细胞介素1β(IL -1β)的变化与肺损伤病理学评分和髓过氧化物酶(MPO)之间的关系。方法48只雄性 SD 大鼠随机平均分成6组:沙漠干热环境轻、中、重度中暑组及其各自常温对照组,然后将6组大鼠分别进行麻醉、固定、置管、接多导生理信号记录仪。3组实验组放置在人工实验舱模拟的沙漠干热环境中,3个对照组放置在常温环境中,干热环境组分别在轻度中暑、中度中暑、重度中暑三个阶段的三个时间点麻醉处死大鼠,并留取血液标本和肺组织,相应对照组取材和时间点同实验组。用 ELISA 法检测大鼠血清炎症因子 TNF -α、IL -6、IL -1β;肺组织测 MPO,应用 HE 染色观察肺组织病理学改变,并根据肺病理组织学变化进行肺损伤病理学评分。结果干热环境中暑各组大鼠肺损伤病理学评分与肺 MPO 及血清炎症因子 TNF -α、IL -6、IL -1β和变化趋势一致,随着中暑程度的增加而逐渐加重,相关性分析提示沙漠干热组肺损伤病理学评分除了和 MPO 密切相关外( P ﹤0.01)还与 TNF -α、IL -6、IL -1β密切相关( P ﹤0.01),常温环境组上述变化不明显,无上述相关性( P ﹥0.05),但与相应的干热环境组差异具有显著性。结论沙漠干热环境下中暑大鼠的肺损伤的发生、发展与炎症因子 TNF -α、IL -6、IL -1β和 MPO 的变化趋势一致,且密切相关,提示沙漠干热环境下炎症因子在沙漠干热环境中暑大鼠的肺损伤过程中起重要作用,MPO 活性是反映沙漠干热环境肺损伤的有用指标。

목적:탐토사막간열배경하중서대서혈청염증인자종류배사인자(TNF -α)、백세포개소6(IL -6)、백세포개소1β(IL -1β)적변화여폐손상병이학평분화수과양화물매(MPO)지간적관계。방법48지웅성 SD 대서수궤평균분성6조:사막간열배경경、중、중도중서조급기각자상온대조조,연후장6조대서분별진행마취、고정、치관、접다도생리신호기록의。3조실험조방치재인공실험창모의적사막간열배경중,3개대조조방치재상온배경중,간열배경조분별재경도중서、중도중서、중도중서삼개계단적삼개시간점마취처사대서,병류취혈액표본화폐조직,상응대조조취재화시간점동실험조。용 ELISA 법검측대서혈청염증인자 TNF -α、IL -6、IL -1β;폐조직측 MPO,응용 HE 염색관찰폐조직병이학개변,병근거폐병리조직학변화진행폐손상병이학평분。결과간열배경중서각조대서폐손상병이학평분여폐 MPO 급혈청염증인자 TNF -α、IL -6、IL -1β화변화추세일치,수착중서정도적증가이축점가중,상관성분석제시사막간열조폐손상병이학평분제료화 MPO 밀절상관외( P ﹤0.01)환여 TNF -α、IL -6、IL -1β밀절상관( P ﹤0.01),상온배경조상술변화불명현,무상술상관성( P ﹥0.05),단여상응적간열배경조차이구유현저성。결론사막간열배경하중서대서적폐손상적발생、발전여염증인자 TNF -α、IL -6、IL -1β화 MPO 적변화추세일치,차밀절상관,제시사막간열배경하염증인자재사막간열배경중서대서적폐손상과정중기중요작용,MPO 활성시반영사막간열배경폐손상적유용지표。
Objective To explore the correlation of the inflammatory cytokine such as TNF - α,IL - 6,IL - 1β with the lung injury and the activity of MPO in the heatstroke rats in the dry heat environment of desert. Methods Forty - eight male SD rats were randomly divided into 6 groups:mild heat stroke group and its control group,moderate heat stroke group and its control group,severe heatstroke group and its control group. After anesthetizing,fixing,intubating,then these rats connected with multichannel physiological recorder. Then the rats of the three experi-mental groups were put into dry heat environment,(temperature 41℃ ,humidity 10% ),and the three control groups were put into normal envi-ronment(temperature 25℃ ,humidity 35% ). After the heatstroke rat model were established,the three heatstroke group were anesthetized and then sacrificed at their corresponding time points(70,110 and 145 min,from the beginning of the experiment),and their corresponding control group were sacrificed at the same time point. Blood samples were taken and lung tissues were harvested,the inflammatory cytokines TNF - α,IL- 6,IL - 1β were detected by ELISA method,the MPO of lung tissue were detected. The optical microscopy was performed to examine the patho-logical changes in lung. Results The cytokines TNF - α,IL - 6,IL - 1β were significantly higher in the dry heat stroke groups than that in their control groups( P ﹤ 0. 01). The TNF - α,IL - 6,IL - 1β,MPO activity and the lung injury pathophysiological score showed higher and higher with the progress of the heat stroke in the groups of dry heat environment of the desert. And each group had significantly different compared with other group in dry heat stroke( P ﹤ 0. 01). The correlation analysis indicated that serum TNF - α,IL - 6,IL - 1β significantly correlated with lung injury pathophysiological score and MPO activity of the lung tissue( P ﹤ 0. 01). The correlation did not emerge in the normal control groups ( P ﹥ 0. 05). Conclusion The serum TNF - α,IL - 6,IL - 1β had the same tendency with lung injury pathophysiological score and MPO ac-tivity of the lung tissue,and they intimately correlated each other,which indicated that the inflammatory cytokine may play an important role in the lung injury in the heat stroke of the dry heat environment of desert. MPO activity may be an useful indicator for the lung injury in such circum-stance.
