目的：分析探讨纳美芬对声带息肉切除术患者静脉全麻诱导时应激激素浓度的影响。方法63例全凭静脉麻醉患者，分为纳美芬组和非纳美芬组，分别于麻醉诱导前（T0）、气管插管前（T1）、气管插管后即刻（T2）、气管插管后5 min（T3）记录收缩压、舒张压、心率，并于各时点采静脉血检测血浆皮质醇（cort）和血管紧张素Ⅱ（ATⅡ）、去甲肾上腺素（NE）和肾上腺素（E）水平。结果麻醉诱导后两组患者HR、SBP、DBP均降低（P<00.05）；T1、T2、T3三个时间点，两组NE，E，cort和AⅡ水平升高（P<0.05）。结论纳美芬能更有效地抑制声带息肉切除术患者静脉全麻诱导时的应激反应。
목적：분석탐토납미분대성대식육절제술환자정맥전마유도시응격격소농도적영향。방법63례전빙정맥마취환자，분위납미분조화비납미분조，분별우마취유도전（T0）、기관삽관전（T1）、기관삽관후즉각（T2）、기관삽관후5 min（T3）기록수축압、서장압、심솔，병우각시점채정맥혈검측혈장피질순（cort）화혈관긴장소Ⅱ（ATⅡ）、거갑신상선소（NE）화신상선소（E）수평。결과마취유도후량조환자HR、SBP、DBP균강저（P<00.05）；T1、T2、T3삼개시간점，량조NE，E，cort화AⅡ수평승고（P<0.05）。결론납미분능경유효지억제성대식육절제술환자정맥전마유도시적응격반응。
ObjectiveTo analyze and discuss the effect of nalmefene on stress hormone concentration of patients with vocal cord polyp resection in TIVA. Methods63 patients with TIVA were divided into nalmefene group and non nalmefene group. The systolic blood pressure, diastolic blood pressure and heart rate were recorded at T0、T1、T2 and T3.Venous blood was colected to detect cortl、ATⅡ、NE and E. Results SBP, DBP and HR were decreased in two groups after anesthesia induction(P< 0.05).At T1、T2、T3, NE,E,cort和AⅡof two groups were significantly elevated (P< 0.05). Conclusion Nalmefene can more effectively inhibit the stress response in patients with vocal cord polyp resection in total intravenous anesthesia.
