目的评价核医学影像诊断多发性骨髓瘤(Multiple Myeloma,MM)的临床价值。方法以单光子发射计算机断层成像术(Single-Photon Emission Computed Tomography,SPECT)诊断62例MM患者,其中42例联合局部X线、CT、MRI检查。结果 SPECT整体敏感度90.32%;59例SPECT阳性患者,浓聚灶75.68%(140/185);42例联合诊断患者,SPECT发现异常病灶142处,CT敏感度、特异度、阳性预测值、阴性预测值、准确率分别为94.74%、87.06%、83.08%、96.10%、90.15%,SPECT则为92.98%、83.53%、79.10%、94.67%、87.32%,均高于X线59.65%、60.00%、50.00%、68.92%、59.86%,MRI敏感度、阳性预测值高于SPECT,差异具有统计学意义(P<0.05);阳性病灶影像学特征与病变大小有关,大面积病灶有软组织肿胀占79.55%(35/44),骨外见组织肿块、尿污染44例。结论 SPECT诊断多发性骨髓瘤效果较高,敏感度与CT/MRI相近,但检查时,需注意排除干扰因素。

ObjectiveTo evaluate the clinical value of nuclear medicine imaging in the diagnosis of multiple myeloma (MM). Methods 62 patients with MM was diagnosed by single photon emission computed tomography, including 42 cases with local X-ray,MRI and CT examination.ResultsThe overal sensitivity of SPECT was 90.32%,59 SPECT positive patients,75.68% concentration range (140/185).42 cases of patients with combined diagnosis, SPECT found 142 lesions.CT sensitivity,specificity,positive predictive value, negative predictive value,accuracy rate were 94.74%、87.06%、83.08%、96.10%、90.15%,MRI’ were 96.49%,97.65%,96.49%,97.65%,97.18%.SPECT’ were 92.98%, 83.53%, 79.10%,94.67%,87.32%.These were higher than that of X-ray’ 59.65%、60.00%、50.00%、68.92%、59.86%.MRI sensitivity,positive predictive value were higher than SPECT,and the difference was statisticaly significant (P< 0.05).Positive lesions imaging characteristics were related to the size of the lesion,and a large area of soft tissue sweling lesions accounted for 79.55% (35/44). It could see the bone tissue mass,44 cases of urinary polution.Conclusion The utility of SPECT in the diagnosis of multiple myeloma is high, and the sensitivity is similar to CT/MRI,but it was necessary to pay attention to the exclusion of interference factors.
