目的：探讨大剂量盐酸氨溴索配合护理干预治疗成人急性呼吸窘迫综合征（ARDS）患者的临床疗效。方法选取2012年7月至2014年7月内江市东兴区人民医院收治的57例ARDS患者资料，按随机数字表法将其分为对照组（27例）和观察组（30例）。对照组患者积极治疗原发病，并行机械通气呼吸支持，试验组患者在对照组基础上给予大剂量盐酸氨溴索治疗，且两组患者均给予积极护理干预。比较两组患者的肺通气指标、氧化应激指标及临床疗效。结果治疗后，试验组患者的内源性呼气末正压（PEEPi）、气道阻力（RAW）均明显低于对照组，氧合指数（PaO2/FiO2）、肺顺应性（CL）均明显高于对照组，丙二醛（MDA）明显低于对照组，谷胱甘肽（GSH）、超氧化物歧化酶（SOD）、总氧化能力（TAO）均明显高于对照组，差异均有统计学意义（均P＜0.05）；观察组患者总有效率为73.3%，明显高于对照组的55.6%，差异有统计学意义（P＜0.05）。结论积极正确的治疗和护理措施是严重ARDS患者抢救成功的重要条件。
목적：탐토대제량염산안추색배합호리간예치료성인급성호흡군박종합정（ARDS）환자적림상료효。방법선취2012년7월지2014년7월내강시동흥구인민의원수치적57례ARDS환자자료，안수궤수자표법장기분위대조조（27례）화관찰조（30례）。대조조환자적겁치료원발병，병행궤계통기호흡지지，시험조환자재대조조기출상급여대제량염산안추색치료，차량조환자균급여적겁호리간예。비교량조환자적폐통기지표、양화응격지표급림상료효。결과치료후，시험조환자적내원성호기말정압（PEEPi）、기도조력（RAW）균명현저우대조조，양합지수（PaO2/FiO2）、폐순응성（CL）균명현고우대조조，병이철（MDA）명현저우대조조，곡광감태（GSH）、초양화물기화매（SOD）、총양화능력（TAO）균명현고우대조조，차이균유통계학의의（균P＜0.05）；관찰조환자총유효솔위73.3%，명현고우대조조적55.6%，차이유통계학의의（P＜0.05）。결론적겁정학적치료화호리조시시엄중ARDS환자창구성공적중요조건。
Objective To investigate the large dose of ambroxol hydrochloride and active nursing intervention in the treatment of adult acute respiratory distress syndrome(ARDS)clinical curative effect.Methods Selected in July 2012 to 2014 July Neijiang City Dongxing District People's Hospital from 57 patients with ARDS patients,according to the random number table method is divided into the control group and treatment group.Patients in the active treatment of the original diseases,paralel mechanical ventilation for respiratory support,patients in the treatment group were given large dose of ambroxol hydrochloride in the treatment group on the basis of control,and two groups of patients were given active nursing intervention,compared two groups of patients with pulmonary ventilation index, oxidative stress index,clinical eficacy.Results After treatment,the test group of patients with endogenous end expiratory positive pressure(PEEPi),airway resistance were significantly lower than those in the control group,oxygenation index (PaO2/ FiO2),lung compliance (CL) were significantly higher than those in the control group, malondialdehyde (MDA) was significantly lower than that of the control group,glutathione(GSH),superoxide dismutase(SOD)and total antioxidant capacity(TAO)were significantly higher than those in the control group, diferences were statisticaly significant(P<0.05); the patients in the observation group total efficiency was 73.3%,significantly higher than that of 55.6% in the control group,the difference was statisticaly significant(P<0.05).Conclusion The positive and correct treatment and nursing measure are the important conditions for the successful rescue of patients with severe ARDS.
