目的:探讨内皮微颗粒作为早期急性心肌缺血评估指标的可能性。方法:将24只雄性 SD 大鼠随机分为假手术组(Sham 组)8只、心肌梗死组(MI 组)8只和心肌缺血再灌注组(I/ R 组)8只,采用冠状动脉结扎制作急性心肌缺血大鼠模型。分别在造模后2 h 和24 h 采外周血检测肌酸激酶同工酶(creatine kinase-MB,CK-MB ),肌钙蛋白 T( cardiac troponin T,cTNT ),内皮微颗粒( endothelial mIcroparticles,EMP),其中循环 EMP 用流式细胞仪检测。通过末端标记法( terminal-deoxynucleoitidyl transferase mediated nick end labeling,TUNEL)检测心肌细胞凋亡情况。结果:造模后2 h 与24 h,I/ R 组大鼠 EMP 水平均显著低于 MI 组(P﹤0.05),造模后2 h MI 组、I/ R 组大鼠的心肌细胞凋亡率均明显高于Sham 组(P﹤0.05)。各组间比较,I/ R 组大鼠的 cTNT 表达水平造模后24 h 较造模后2 h 显著下降,差异有统计学意义(P﹤0.05);MI 组大鼠的 CK-MB 表达水平造模后24 h 较造模后2 h 显著性升高(P﹤0.05);而 EMP 与心肌细胞凋亡率各组间比较差异无统计学意义。结论:内皮微颗粒可以作为早期急性心肌缺血的评估指标。

목적:탐토내피미과립작위조기급성심기결혈평고지표적가능성。방법:장24지웅성 SD 대서수궤분위가수술조(Sham 조)8지、심기경사조(MI 조)8지화심기결혈재관주조(I/ R 조)8지,채용관상동맥결찰제작급성심기결혈대서모형。분별재조모후2 h 화24 h 채외주혈검측기산격매동공매(creatine kinase-MB,CK-MB ),기개단백 T( cardiac troponin T,cTNT ),내피미과립( endothelial mIcroparticles,EMP),기중순배 EMP 용류식세포의검측。통과말단표기법( terminal-deoxynucleoitidyl transferase mediated nick end labeling,TUNEL)검측심기세포조망정황。결과:조모후2 h 여24 h,I/ R 조대서 EMP 수평균현저저우 MI 조(P﹤0.05),조모후2 h MI 조、I/ R 조대서적심기세포조망솔균명현고우Sham 조(P﹤0.05)。각조간비교,I/ R 조대서적 cTNT 표체수평조모후24 h 교조모후2 h 현저하강,차이유통계학의의(P﹤0.05);MI 조대서적 CK-MB 표체수평조모후24 h 교조모후2 h 현저성승고(P﹤0.05);이 EMP 여심기세포조망솔각조간비교차이무통계학의의。결론:내피미과립가이작위조기급성심기결혈적평고지표。
Objective:To investigate the possibility of endothelial microparticles( EMP)being used as evaluation index for early acute myocardial ischemia.Methods:Twenty-four male SD rats were randomly divided into sham operation group,myocardial infarction( MI)group,and ischemia reperfusion( I/ R)group( n = 8 each). The rat models with acute myocardial ischemia were prepared by coronary artery ligation. At 2h and 24h after modeling,peripheral blood was taken to measure creatine kinase-MB( CK-MB),cardiac troponin T (cTNT),and EMP. The circulating level of EMP were measured by flow cytometry. The myocardial apoptosis was determined by terminal-deoxynucleoitidyl transferase mediated nick end labeling(TUNEL)assay.Results:At 2h and 24 h after modeling,the EMP level in I/ R group was significantly lower than that in MI group(P﹤0.05). At 2 h after modeling,the myocardial apoptosis in MI group and I/ R group was significantly higher than that in Sham operation group(P﹤ 0. 05). The expression level of cTNT in I/ R group significantly decreased at 24 h after modeling compared with that at 2h after modeling,with statistical difference(P﹤0.05). The expression level of CK-MB in MI group significantly increased at 24h after modeling compared with that at 2 h after modeling(P﹤0. 05). There were no statistical differences in EMP and cardiomyocyte apoptosis between different groups. Conclusion:EMP can be used as evaluation index for early acute myocardial ischemia.
