目的:探讨关节滑液血管活性肠肽水平与膝前交叉韧带( ACL)损伤后关节软骨退变的相关性。方法选取来自本组进行ACL重建的患者63例,术前抽取患者的关节滑液,并对患者膝关节损伤情况进行关节镜下的Noyes评分,血管活性肠肽( VIP)水平,关节软骨退变相关因子Ⅱ型胶原C端肽( CTX-Ⅱ)、聚集蛋白聚糖-1( AGG-1)、聚集蛋白聚糖-2( AGG-2)水平以及炎症因子基质金属蛋白酶-3(MMP-3),白介素-6(IL-6)水平采用酶联免疫吸附实验(ELISA)进行检测。结果随着ACL损伤后时间延长,Noyes评分明显升高。关节滑液VIP水平与Noyes评分具有显著的负相关性(rs=-0.692,P<0.01);VIP水平与关节滑液退变因子CTX-Ⅱ(r=-0.546,P<0.01),AGG-1(r=-0.598,P<0.01),AGG-2(r=-0.597,P<0.01)均具有显著的负相关性;关节滑液VIP水平与MMP-3(r=-0.382,P<0.05)和IL-6(r=-0.301,P<0.05)亦具有显著的负相关性。结论 VIP低表达与ACL损伤后关节软骨退变程度、骨关节炎病程进展有关,VIP可作为关节软骨损伤进展的生物标志物。

목적:탐토관절활액혈관활성장태수평여슬전교차인대( ACL)손상후관절연골퇴변적상관성。방법선취래자본조진행ACL중건적환자63례,술전추취환자적관절활액,병대환자슬관절손상정황진행관절경하적Noyes평분,혈관활성장태( VIP)수평,관절연골퇴변상관인자Ⅱ형효원C단태( CTX-Ⅱ)、취집단백취당-1( AGG-1)、취집단백취당-2( AGG-2)수평이급염증인자기질금속단백매-3(MMP-3),백개소-6(IL-6)수평채용매련면역흡부실험(ELISA)진행검측。결과수착ACL손상후시간연장,Noyes평분명현승고。관절활액VIP수평여Noyes평분구유현저적부상관성(rs=-0.692,P<0.01);VIP수평여관절활액퇴변인자CTX-Ⅱ(r=-0.546,P<0.01),AGG-1(r=-0.598,P<0.01),AGG-2(r=-0.597,P<0.01)균구유현저적부상관성;관절활액VIP수평여MMP-3(r=-0.382,P<0.05)화IL-6(r=-0.301,P<0.05)역구유현저적부상관성。결론 VIP저표체여ACL손상후관절연골퇴변정도、골관절염병정진전유관,VIP가작위관절연골손상진전적생물표지물。
Objective To investigate the relationship between synovial fluid vasoactive intestinal peptide ( VIP ) levels and cartilage damage degree in patients with anterior cruciate ligament ( ACL ) injury.Methods Sixty-three patients diagnosed as ACL injury were admitted to our hospital and enrolled . Synovial fluid was obtained before the surgery .The Noyes score method was used to assess the articular cartilage damage arthroscopically .VIP levels, cartilage degradation biomarkers such as collagen type Ⅱ(CTX-Ⅱ),aggrecan-1(AGG-1), and aggrecan-2 (AGG-2), as well as inflammation markers interleukin-6 (IL-6) and (matrix metalloproteinases-3)MMP-3 in the synovial fluid were determined by enzyme-linked immunosorbent assay ( ELISA ) .Results The articular cartilage in ACL deficiency patients deteriorated significantly with time after the injury .VIP levels in synovial fluid showed negative correlations with Noyes socres (rs =-0.692,P<0.01), CTX-Ⅱ(r=-0.546,P<0.01), AGG1(r=-0.598,P<0.01), AGG2 (r=-0.597,P<0.01), as well as MMP-3 (r=-0.382,P<0.05) and IL-6 (r=-0.301, P<0.05) .Conclusion VIP levels in synovial fluid present an independent and negative correlation with articular cartilage degeneration and osteoarthirtis progression in patients with ACL injury;VIP may serve as a biomarker for reflecting the cartilage damage .
