目的:探讨腔隙性脑梗死(CLI)与血压变异性、颈动脉粥样硬化的关系。方法96例原发性高血压并发 CLI 患者为观察组,88例原发性高血压患者为对照组。全部患者行头颅 CT 或 MRI 检查、动态血压监测、颈动脉超声检查,并进行颈动脉硬化分型及等级积分。结果年龄、性别、吸烟史、高血压、糖尿病、24h 收缩压标准差(24hSBPsd)、总胆固醇、高密度脂蛋白胆固醇及颈动脉硬化积分均与脑梗死有显著相关关系,经多因素 Logistic 回归分析显示,24h SBPsd(r=0.4832,P<0.001)和颈动脉硬化积分(r=0.4960,P<0.001)与脑梗死呈显著正相关。结论24hSBPsd、颈动脉硬化与脑梗死密切相关,是脑梗死发生发展的有效预测指标。

목적:탐토강극성뇌경사(CLI)여혈압변이성、경동맥죽양경화적관계。방법96례원발성고혈압병발 CLI 환자위관찰조,88례원발성고혈압환자위대조조。전부환자행두로 CT 혹 MRI 검사、동태혈압감측、경동맥초성검사,병진행경동맥경화분형급등급적분。결과년령、성별、흡연사、고혈압、당뇨병、24h 수축압표준차(24hSBPsd)、총담고순、고밀도지단백담고순급경동맥경화적분균여뇌경사유현저상관관계,경다인소 Logistic 회귀분석현시,24h SBPsd(r=0.4832,P<0.001)화경동맥경화적분(r=0.4960,P<0.001)여뇌경사정현저정상관。결론24hSBPsd、경동맥경화여뇌경사밀절상관,시뇌경사발생발전적유효예측지표。
Objective To investigate the relationship between cerebral lacunar infarction (CLI) and blood pressure variability, carotid arterial atherosclerosis.Methods 96 patients with cerebral lacunar infarction and essential hypertension and 88 patients with essential hypertension (NCLI) were analyzed.Demographic features and clinical data of all patients were recorded.Systemic evaluations including whole blood chemical test,brain CT/MRI, ambulatory blood pressure measurement(ABPM) and carotid artery ultrasonography were performed in CLI and NCLI patients,and then carotid artery scores and the type of carotid artery atherosclerosis were involved.Statistical analysis was done by SPSS 13.0 software.Results The age, sex, smoking history, longer hypertensive history, T2DM, 24hSBPsd,TC, HDL-C and carotid artery scores were significantly related with cerebral infarction.Multivariate regression analysis showed that 24hSBPsd(r=0.4832,P<0.001) and carotid artery scores (r=0.4960,P<0.001)were significantly related with cerebral infarction.Conclusion 24hSBPsd and carotid artery score are significantly related with cerebral infarction, and those can be used to predict the development of cerebral infarction.
