目的：探讨 Le FortⅠ型骨切开术在矫治陈旧性面中部骨折所致颌骨畸形的应用价值。方法对42例陈旧性面中部骨折所致颌骨畸形的患者，通过螺旋 CT 及三维重建、模型外科分析、咬导板制作等准备，术中采用 Le FortⅠ型骨切开术对本组病例进行治疗，观察患者术后面部畸形及咬关系的恢复情况，分析 Le FortⅠ型骨切开术对颌骨畸形以及咬关系的矫治效果。结果本组42例患者中39例的面部形态和咬关系恢复良好，有3例患者术后咬关系欠佳，经正畸治疗后恢复正常，无严重手术并发症发生。结论 Le FortⅠ型骨切开术是治疗陈旧性面中部骨折所致颌骨畸形的有效方法，可取得满意的治疗效果，值得在临床中进一步推广应用。
목적：탐토 Le FortⅠ형골절개술재교치진구성면중부골절소치합골기형적응용개치。방법대42례진구성면중부골절소치합골기형적환자，통과라선 CT 급삼유중건、모형외과분석、교도판제작등준비，술중채용 Le FortⅠ형골절개술대본조병례진행치료，관찰환자술후면부기형급교관계적회복정황，분석 Le FortⅠ형골절개술대합골기형이급교관계적교치효과。결과본조42례환자중39례적면부형태화교관계회복량호，유3례환자술후교관계흠가，경정기치료후회복정상，무엄중수술병발증발생。결론 Le FortⅠ형골절개술시치료진구성면중부골절소치합골기형적유효방법，가취득만의적치료효과，치득재림상중진일보추엄응용。
Objective To investigate the effects of Le FortⅠ osteotomy in the treatment of jaw deformities caused by midfacial old fractures. Methods 42 cases of midfacial old fractures with jaw deformities were treated by Le FortⅠ osteotomy based on preoperative Spiral CT and 3D reconstruction. Model surgery was performed on occlusal guide preoperatively. The rehabilitation of facial deformity and occlusion was observed postoperatively. The effects of Le FortⅠ osteotomy for correcting midfacial old fractures were analyzed. Results The facial morphology and occlusion of 39 cases among 42 patients recovered well. Among which, 3 cases had postoperative malocclusion, but healed completely after orthodontic treatment. There were no severe complications happened postoperatively in all cases. Conclusion Le FortⅠ osteotomy is an important and effective method for correcting jaw deformities caused by midfacial old fractures.
