目的:运用磁共振扩散张量成像(DTI)序列扫描,并用基于体素的分析(VBA)方法对阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者全脑不同脑区的脑白质神经纤维束的完整性进行检测,探讨 DTI对 OSAHS 患者脑白质损害的评价。方法收集 OSAHS 患者26例,以及与 OSAHS 组年龄、性别相匹配的正常志愿者22例,两组均为男性,行头颅 DTI 扫描。用 DTI Studio 软件和基于 Matlab 平台的 SPM8和 REST 软件对所采集的图像进行后处理,并通过 VBA 方法分析。观察 OSAHS 组各向异性分数(FA)值相对于正常对照组的变化,得出两组差异有统计学意义的脑区位置以及簇体积。结果与对照组相比较,OSAHS 组部分脑区 FA 值显著减低(P<0.001并经过 alphasim 校正),FA 值下降的区域主要位于额叶及顶叶皮质、前、后扣带回、脑干、腹外侧核、胼胝体、背侧丘脑及杏仁核等。结论OSAHS 患者多个脑区 FA 值均较正常志愿者明显减低,证实 OSAHS 患者脑白质存在着广泛的损伤。运用 VBA 方法对 DTI 分析可以评价 OSAHS 患者的脑白质损伤。

목적:운용자공진확산장량성상(DTI)서렬소묘,병용기우체소적분석(VBA)방법대조새성수면호흡잠정저통기종합정(OSAHS)환자전뇌불동뇌구적뇌백질신경섬유속적완정성진행검측,탐토 DTI대 OSAHS 환자뇌백질손해적평개。방법수집 OSAHS 환자26례,이급여 OSAHS 조년령、성별상필배적정상지원자22례,량조균위남성,행두로 DTI 소묘。용 DTI Studio 연건화기우 Matlab 평태적 SPM8화 REST 연건대소채집적도상진행후처리,병통과 VBA 방법분석。관찰 OSAHS 조각향이성분수(FA)치상대우정상대조조적변화,득출량조차이유통계학의의적뇌구위치이급족체적。결과여대조조상비교,OSAHS 조부분뇌구 FA 치현저감저(P<0.001병경과 alphasim 교정),FA 치하강적구역주요위우액협급정협피질、전、후구대회、뇌간、복외측핵、변지체、배측구뇌급행인핵등。결론OSAHS 환자다개뇌구 FA 치균교정상지원자명현감저,증실 OSAHS 환자뇌백질존재착엄범적손상。운용 VBA 방법대 DTI 분석가이평개 OSAHS 환자적뇌백질손상。
Objective To detect the integrality of brain white matter fibers in different brain regions, and to discuss brain white matter lesion in patients with obstructive sleep apnea-hypopnea syndrome ( OSAHS) using diffusion tensor imaging (DTI) and voxel-based analysis (VBA) method.Methods Twenty-six male patients with OSAHS and twenty-two male healthy controls matched for age and gender, were included in this study.Two groups were performed conventional brain DTI scan.The DTI data were processed with the DTI studio, the software of REST and SPM8 on the Matlab.The date were analysed with VBA.Next, we can get the changes of fractional anisotmpy (FA) value in the OSAHS, compared to control subjects.And at last, it is concluded that the brain areas and cluster sizes with significant difference between the two groups.Results Compared with the control group, the brain area FA value of experimental group were significantly decreased, the FA values were significantly decreased in the front gyrus, parietal lobule, anterior cingulate, posterior cingulate, brainstem, ventral lateral nucleus, corpus callosum, amygdala and so on.Conclusion The FA values of multiple brain regions with OSAHS patients are significantly lower than those in normal volunteers, which confirmed that the white matter has a wide range of damage.The white matter microstructure can be qualitative analyzed using DTI based on the VBA method, which can evaluate brain white matter lesion in OSAHS patiens.
