目的:分析海洋石油工业湛江基地职工脑卒中发病的主要危险因素,为制定脑卒中的预防和控制措施提供科学依据。方法对117例脑卒中住院患者(脑卒中组)和117例非脑卒中者(对照组)进行对照研究,利用统一的《健康调查表》,对收集的所有对象的相关资料进行调查,包括性别、年龄、体质量、长期海上工作年限、吸烟史、饮酒情况、血压、糖尿病史、心脏病史、心理压力、胆固醇、三酰甘油、低密度脂蛋白、高密度脂蛋白、同型半胱氨酸等,结合流行病学调查研究方法以及多种统计方法,分析海洋石油工业湛江基地职工脑卒中主要危险因素。结果多因素非条件Logistic回归分析显示,海洋石油工业湛江基地职工脑卒中相关因素OR值及95%CI值分别为:年龄2.052、1.041~5.063,高血压2.108、1.104~5.152,吸烟2.452、1.156~5.607,糖尿病1.053、1.031~3.077,心脏病史1.965、1.105~4.344,体质量指数2.353、1.120~4.675,心理压力大1.854、1.091~7.501,长期海上工作1.974、1.124~4.351,饮酒0.837、0.784~0.996,同型半胱氨酸血症1.221、1.092~3.325,低密度脂蛋白胆固醇1.536、1.105~2.227,高密度脂蛋白胆固醇0.313、0.092~1.025。结论年龄、高血压、吸烟、糖尿病、心脏病史、心理压力、长期海上工作、总胆固醇、三酰甘油、低密度脂蛋白胆固醇、同型半胱氨酸是脑卒中的危险因素,高密度脂蛋白是脑卒中的保护因素,饮酒不是脑卒中的危险因素。

Objective To explore main risk factors associated with stroke among workers in Zhanjiang Base of Offshore Oil Industry , so as to provide scientific evidence for the development of preventive and control measures against stroke .Methods A comparative study was made by using 117 stroke inpatients and the same number of non-stroke patients .By using the same health survey questionnaire , relative medical data , including gender, age, body mass, yeas of service at sea, smoking, alcoholism, blood pressure, diabetes, heart disease, mental stress, cholesterol, triglyceride, low density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol ( HDL-C ) and homocysteine were collected and analyzed .Multiple-factor unconditional Logistic regression analysis was applied to evaluate major risk factors associated with stroke among workers in Zhanjiang Base of Offshore Oil Industry .Results Multiple-factor unconditional Logistic regression analysis showed that the OR value and 95%CI for age were (2.052, 1.041-5.063), hypertension (2.108, 1.104-5.152), smoking (2.452, 1.156 -5.607), diabetes (1.053, 1.031 -3.077), heart disease (1.965, 1.105-4.344), high body mass index ( 2.353, 1.120-4.675), high mental stress (1.854, 1.091-7.501), years of service at sea (1.974, 1.124 -4.351), alcoholism (0.837, 0.784 -0.996), homocysteine (1.221, 1.092-3.325), low-density lipoprotein (1.536, 1.105 -2.227) and high-density lipoprotein (0.313, 0.092-1.025).Conclusions Research results indicated that age , hypertension, smoking, diabetes, heart disease, high mental stress, long years of service at sea, high body mass index, low-density lipoprotein, homocysteine were all risk factors associated with stroke , high-density lipoprotein cholesterol was a protective factor and alcoholism was not a risk factor of stroke .
