目的:探讨256层3D CT 血管成像(3D - CTA)对颅内微小动脉瘤(IMA)的诊断价值。方法:回顾性研究3D - CTA 及3D - DSA 诊断的38例(40个)颅内微小动脉瘤,以3D - DSA 为标准,比较两种检查对动脉瘤数目、位置、大小、形态、瘤颈、子瘤显示的差异。结果:3D - CTA 显示38个微小动脉瘤,漏诊2例2个,2例3D - CTA 显示单个动脉瘤,3D - DSA 证实为2个动脉瘤。3D - CTA 显示2例假阳性。颈内动脉7个,基底动脉环18个,大脑前动脉8个,大脑中动脉7个。CTA 与 DSA 两者均能清楚显示 IMA 特征性征象情况。结论:256层3D - CTA 对颅内微小动脉瘤显示清晰,与3D - DSA 检出无明显差异。256层3D - CTA 可以作为诊断颅内微小动脉瘤的首选影像学方法。

목적:탐토256층3D CT 혈관성상(3D - CTA)대로내미소동맥류(IMA)적진단개치。방법:회고성연구3D - CTA 급3D - DSA 진단적38례(40개)로내미소동맥류,이3D - DSA 위표준,비교량충검사대동맥류수목、위치、대소、형태、류경、자류현시적차이。결과:3D - CTA 현시38개미소동맥류,루진2례2개,2례3D - CTA 현시단개동맥류,3D - DSA 증실위2개동맥류。3D - CTA 현시2례가양성。경내동맥7개,기저동맥배18개,대뇌전동맥8개,대뇌중동맥7개。CTA 여 DSA 량자균능청초현시 IMA 특정성정상정황。결론:256층3D - CTA 대로내미소동맥류현시청석,여3D - DSA 검출무명현차이。256층3D - CTA 가이작위진단로내미소동맥류적수선영상학방법。
Objective:To investigate the difference of 256 slice 3 - dimension computed tomography angiography(3D -CTA)and 3 - dimension digital subtraction angiography(3D - DSA)in diagnosis of intracranial micro - aneurysms(IMA). Methods:To retrospectively research 38 patients(40 ovarian)with IMA,all patients were examined in both 3D - CTA and 3D- DSA. With 3D - DSA as standard,The informations of IMA about number,location,size,shape,aneurysm neck,children an-eurysms were compared between two examined. Results:Thirty - eight ovarian with IMA were found by 3D - CTA,false - nega-tive findings by 3D - CTA in 2 patient was identified having 2 IMA by 3D - DSA,while only one IMA showed by 3D - CTA. False positive sign of IMA was showed by 3D - CTA in 2 case. The location of IMA was in the internal carotid artery in 7,in the Cerebral arterial circle in 18,in the anterior cerebral artery in 8,in the middle cerebral artery in 7. The characteristic mani-festations of IMA can dispay by 3D - CTA between 3D - DSA. Conclusion:3D - CTA can dispay IMA clearly,and was no sig-nificant difference in accuracy of detected about IMA by 3D - DSA. we suggest that 3D - CTA may be the first choice for diag-nosis of IMA.
