目的：探讨彩色多普勒超声在精索静脉曲张高位结扎术后随访中的价值。方法选择63例经临床确诊为左精索静脉曲张并行精索静脉曲张高位结扎术的患者，分别用彩色多普勒超声测量术前、术后6个月睾丸包膜动脉血流参数，测值包括：收缩期峰值速度（ PSV）、舒张末期血流速度（ EDV）、阻力指数（ RI）；同时，对比手术前后精液质量，包括：精子总数、活率、精子密度。结果与术前相比，术后患者精液质量明显提高（ P ＜0．05）；术后6个月左侧睾丸包膜动脉PSV、RI下降（ P ＜0．05），EDV差别无统计学意义（ P ＞0．05）；术后右侧睾丸包膜动脉PSV、EDV、RI差异无统计学意义（ P ＞0．05）。相关分析显示，左侧睾丸包膜动脉PSV、RI与精子密度相关（ P ＜0．05）。结论精索静脉曲张高位结扎可有效改善睾丸血供及精液质量；彩色超声多普勒睾丸包膜动脉阻力指数检测可作为精索静脉曲张术后随访的重要指标。
목적：탐토채색다보륵초성재정색정맥곡장고위결찰술후수방중적개치。방법선택63례경림상학진위좌정색정맥곡장병행정색정맥곡장고위결찰술적환자，분별용채색다보륵초성측량술전、술후6개월고환포막동맥혈류삼수，측치포괄：수축기봉치속도（ PSV）、서장말기혈류속도（ EDV）、조력지수（ RI）；동시，대비수술전후정액질량，포괄：정자총수、활솔、정자밀도。결과여술전상비，술후환자정액질량명현제고（ P ＜0．05）；술후6개월좌측고환포막동맥PSV、RI하강（ P ＜0．05），EDV차별무통계학의의（ P ＞0．05）；술후우측고환포막동맥PSV、EDV、RI차이무통계학의의（ P ＞0．05）。상관분석현시，좌측고환포막동맥PSV、RI여정자밀도상관（ P ＜0．05）。결론정색정맥곡장고위결찰가유효개선고환혈공급정액질량；채색초성다보륵고환포막동맥조력지수검측가작위정색정맥곡장술후수방적중요지표。
Objectives To investigate the value of Color Doppler Ultrasound in the follow -up of Varicoce-lectomy.Methods A total of 63 patients clinically diagnosed with left varicocele who underwent varicocelectomy were examined.The testicular capsular artery blood flow parameters(peak systolic, end diastolic velocity, resistive in-dex) were examined before and 6 months after the operation by Color Doppler Ultrasound.We also evaluated preoper-ative and postoperative semen parameters( sperm density,sperm motility rate, total motile sperm count) .Results The semen quality increased significantly after surgery( P <0.05).Peak systolic and resistive index of the left testic-ular capsular artery decreased significantly in six months after the operations( P <0.05),while end diastolic velocity no significant difference.No significant difference was detected between the preoperative and postoperative blood flow parameters in the right testicular capsular artery( P >0.05).The pearson correlation analysis showed that there were significant correlation between sperm density and PSV,RI in the left testicular capsular artery( P <0.05) .Con-clusions Our data shows that a significant improvement occurs in testicular blood and semen quality after varicoc-electomy.The detected of RI with Color Doppler Ultrasound may be a great data in the follow -up of Varicocelecto-my.
