目的:探讨p57、p53和p63蛋白在鉴别诊断水肿性流产(HA),部分性葡萄胎(PHM)和完全性葡萄胎(CHM)的价值。方法:采用免疫组织化学染色法检测HA(n=21)、PHM(n=28)和CHM(n=32)石蜡组织切片中p57、p53和p63的表达。结果:HA、PHM和CHM组织中,p57蛋白的阳性表达率分别为90.5%(19/21)、100%(28/28)和0.0%(0/32),PHM和CHM组间有明显差异(P<0.01);p53蛋白的阳性表达率分别为28.3%(5/21)、71.4%(20/28)和87.5%(28/32),HA、PHM组相比也有明显差异(P<0.01);p63蛋白的阳性表达率分别为81.0%(17/21)、89.3%(25/28)和87.5%(28/32),各组间均无统计学差异。结论:以常规石蜡切片的病理形态特征为基础,结合免疫组化检测p57、p53和p63的结果,对鉴别水肿性流产和葡萄胎具有重要价值。

Objective:To investigate the value of p57 , p53 and p63 proteins in distinguishing diagnosis of hydropic abortion (HA), partial hydatidiform mole (PHM) and complete hydatidiform mole (CHM). Methods:Immunohistochemical stains (Envision method) for p57, p53 and p63 were performed in tissue samples of HA (n=21), PHM (n=28) and CHM (n=32). Results:The positive rates of p57 in HA, PHM and CHM were 90.5%(19/21),100%(28/28) and 0%(0/32), respectively. There was a significant difference between PHM and CHM (P<0.01). The positive rates of p53 in HA and PHM were 28.3%(5/21) and 71.4%(20/28) and 87.5%(28/32) respectively, and the difference between them was statistically significant(P<0.01). The p63 expression in HA was less than that in HM (PHM and CHM), and the rates were 81%(17/21) and 89.3%(25/28) and 87.5%(28/32). The expressions were not significantly different between HA and HM. Conclusion: In pathological morphological characteristics as the basis, combined with immunohistochemical detection of p57, P53 and P63, and has important value for distinguishing hydropic abortion and hydatidiform mole.
