目的：回顾性分析影响百草枯中毒患者生存及肾脏的预后因素。方法回顾性分析2010年1月～2014年5月在平凉市医院及本院就诊的百草枯中毒患者的临床资料。所有患者随访至死亡或中毒后3个月，根据患者是否存活分为存活组和死亡组，并根据存活组患者的肾损伤情况分为急性肾损伤和慢性肾损伤。分别比较两组患者各项临床指标和AKIN分期；多因素分析采用Cox回归模型，对影响生存患者预后效果的因素进行分析。结果存活组患者和死亡患者两组患者的年龄、是否血液净化、AKIN分期、服毒量和入院时血糖、中性细胞比例、白细胞计数、尿素氮、血清肌酐、血K＋、阴离子间隙等检测指标比较差异均具有统计学意义（P＜0．05），且其中AKIN分期、服毒量是影响患者预后的独立预后因素。 AKIN3期患者死亡率明显高于AKIN2期和AKIN1期和肾功能正常者。结论影响患者百草枯中毒患者的生存状况的主要因素为服毒量、AKI、CO2总量、AKIN分期。
목적：회고성분석영향백초고중독환자생존급신장적예후인소。방법회고성분석2010년1월～2014년5월재평량시의원급본원취진적백초고중독환자적림상자료。소유환자수방지사망혹중독후3개월，근거환자시부존활분위존활조화사망조，병근거존활조환자적신손상정황분위급성신손상화만성신손상。분별비교량조환자각항림상지표화AKIN분기；다인소분석채용Cox회귀모형，대영향생존환자예후효과적인소진행분석。결과존활조환자화사망환자량조환자적년령、시부혈액정화、AKIN분기、복독량화입원시혈당、중성세포비례、백세포계수、뇨소담、혈청기항、혈K＋、음리자간극등검측지표비교차이균구유통계학의의（P＜0．05），차기중AKIN분기、복독량시영향환자예후적독립예후인소。 AKIN3기환자사망솔명현고우AKIN2기화AKIN1기화신공능정상자。결론영향환자백초고중독환자적생존상황적주요인소위복독량、AKI、CO2총량、AKIN분기。
Objectives To explore Retrospective of influencing factors of patient survival and renal progno-sis of paraquat poisoning.Methods A retrospective analysis on patients with the clinical data from January 2010 to May 2014 treated in our hospital and Pingliang Hospital.After 3 months, all patients were followed until death or after the poisoning.According to whether survived ,patients were divided into survival group and death group.Mean-while according to damage level of renal ,the survival of renal injury group was divided into acute renal injury and chronic kidney injury.Respectively Comparing two groups of patients with clinical indexes and AKIN stage ,using Cox regression model analysis in multivariate analysis to analysisthe influence factors on the prognosis factors of survival <br> patients.Results Aage, the amount of poison,if blood purification,AKIN staging and neutrophil proportion,into the school white blood cell count,blood glucose,blood urea nitrogen,serum creatinine,blood K+, anion gap differ-ences were statistically significant( P <0.05)between the two groups, and AKIN stage, the amount of poison indi-vidualy affect the prognosis of the patients.Mortality in patients with AKIN3 were higher than that in AKIN2 phase and AKIN1 phase and normal renal function people.Conclusions The main factors affecting the survival status of patients with paraquat poisoning is the amount of poison,AKI,total CO2 ,AKIN staging.
