目的:探讨氯己定龈下冲洗辅助牙周基础治疗对冠心病伴牙周炎患者的效果及对血清IL-10和TNF-α水平的影响。方法选择冠心病伴中重度慢性牙周炎患者62例,所有患者均给予牙周基础治疗,包括龈上洁治、口腔健康指导、龈下刮治术及根面平整术,同时给予氯己定龈下冲洗辅助治疗。比较治疗前后探针深度、牙周附着丧失、龈沟出血指数、白介素10(IL-10)、肿瘤坏死因子α(TNF-α)水平。结果治疗后探针深度、龈沟出血指数和牙周附着丧失低于治疗前,差异有统计学意义(P<0.05),治疗后IL-10水平高于治疗前,TNF-α水平低于治疗前,差异有统计学意义(P<0.05)。结论氯己定龈下冲洗辅助牙周基础治疗冠心病伴牙周炎可使牙周指标得到改善,IL-10水平升高,TNF-α水平降低,有利于冠心病的预防、治疗及延缓疾病的进程。

목적:탐토록기정간하충세보조아주기출치료대관심병반아주염환자적효과급대혈청IL-10화TNF-α수평적영향。방법선택관심병반중중도만성아주염환자62례,소유환자균급여아주기출치료,포괄간상길치、구강건강지도、간하괄치술급근면평정술,동시급여록기정간하충세보조치료。비교치료전후탐침심도、아주부착상실、간구출혈지수、백개소10(IL-10)、종류배사인자α(TNF-α)수평。결과치료후탐침심도、간구출혈지수화아주부착상실저우치료전,차이유통계학의의(P<0.05),치료후IL-10수평고우치료전,TNF-α수평저우치료전,차이유통계학의의(P<0.05)。결론록기정간하충세보조아주기출치료관심병반아주염가사아주지표득도개선,IL-10수평승고,TNF-α수평강저,유리우관심병적예방、치료급연완질병적진정。
Objective To investigate the efficacy of subgingival Chlorhexidine rinse aiding periodontal basic treatment in patients of coronary artery disease with periodontitis and its effect on serum IL-10 and TNF-α levels. Methods 62 cases of stable coronary artery disease with moderate to severe chronic periodontitis patients were collected,all patients were given base periodontal treatment,including subgingival scaling therapy,oral health guidance,curettage and root planing subgingival,and were given subgingival Chlorhexidine rinse under adjuvant therapy at the same time.The probe depth,periodontal attachment loss,sulcus bleeding index,IL-10,TNF-αlevels of patients were compared before and after the treatment. Results The probe depth,sulcus bleeding index and periodontal attachment loss of patient after treatment were lower than prior treatment.The differences were statistically significant (P<0.05).IL-10 level was higher than prior treatment.TNF-αlevel was lower than prior treatment,the differences were statistically significant (P<0.05). Conclusion Subgingival Chlorhexidine rinse aiding periodontal basic treatment in patients of coronary artery disease with periodon-titis can significantly improve periodontal index,elevate IL-10 levels,lower TNF-αlevel,and is conducive to the preven-tion,treatment and delaying of coronary heart disease.
