目的：探讨血清Dickkopf-1(DKK-1)蛋白联合CA199检测在胃癌诊断中的应用价值。方法选取2013年5月~2014年12月本院收治的58例原发性胃癌患者作为胃癌组，同期收治的60例慢性浅表性胃炎患者作为慢性胃炎组，同时收集60例体检健康人作为健康对照组。3组同时应用酶联免疫吸附法测定DKK-1，应用化学发光法测定CA199，并比较DKK-1、CA199诊断胃癌的特异度、敏感度、准确度、阳性似然比、阴性似然比。结果与慢性胃炎组和健康对照组相比，胃癌组血清DKK-1、CA199水平显著升高(P<0.01)；与单独应用DKK-1或CA199检测相比，DKK-1联合CA199检测的敏感度、准确度、阳性似然比明显升高(P<0.05)，阴性似然比明显下降(P<0.05)。结论血清DKK-1可以作为胃癌临床辅助诊断的依据之一，血清DKK-1联合CA199检测对提高胃癌的诊断水平有独特的优势，值得临床推广应用。
목적：탐토혈청Dickkopf-1(DKK-1)단백연합CA199검측재위암진단중적응용개치。방법선취2013년5월~2014년12월본원수치적58례원발성위암환자작위위암조，동기수치적60례만성천표성위염환자작위만성위염조，동시수집60례체검건강인작위건강대조조。3조동시응용매련면역흡부법측정DKK-1，응용화학발광법측정CA199，병비교DKK-1、CA199진단위암적특이도、민감도、준학도、양성사연비、음성사연비。결과여만성위염조화건강대조조상비，위암조혈청DKK-1、CA199수평현저승고(P<0.01)；여단독응용DKK-1혹CA199검측상비，DKK-1연합CA199검측적민감도、준학도、양성사연비명현승고(P<0.05)，음성사연비명현하강(P<0.05)。결론혈청DKK-1가이작위위암림상보조진단적의거지일，혈청DKK-1연합CA199검측대제고위암적진단수평유독특적우세，치득림상추엄응용。
Objective To explore the application value of serum Dickkopf-1 (DKK-1) protein combined with CA199 test in the diagnosis of gastric cancer. Methods 58 patients with primary gastric cancer admitted into our hospital from May 2013 to December 2014 were selected as gastric cancer group,60 patients with chronic superficial gastritis in the same time were selected as chronic gastritis group,and 60 cases of medical healthy people were selected as healthy control group.In the 3 groups,ELISA was applied to test DKK-1and chemiluminescence was applied to test CA199.The specificity,sensitivity,accuracy,positive likelihood ratio and negative likelihood ratio of DKK-1 and CA199 was com-pared. Results Compared with chronic gastritis group and healthy control group,levels of serum DKK-1 and CA199 in p gastric cancer group significantly improved (P<0.01).Compared with the single application of DKK-1 or CA199,the sensitivity,accuracy and positive likelihood ratio of DKK-1 combined with CA199 significantly increased (P<0.05),and the negative likelihood ratio significantly decreased (P<0.05). Conclusion Serum DKK-1 can be used as a basis for clinical diagnosis of gastric cancer.Serum DKK-1 combined with CA199 test detection has a unique advantage in the diagnosis of gastric cancer,it is worthy of clinic application.
