目的 探讨胆道闭锁(biliary atresia,BA)患儿肝组织中转移相关基因3(MTA3)、锌指转录因子Snail1、E-钙黏素(E-cadherin)三者的表达、相关性及其在胆道闭锁发生、发展中的作用.方法 选取2012年2月至2013年9月河北医科大学第二医院小儿外科收治的21例胆道闭锁、17例胆总管囊肿及10例肝破裂患儿的肝组织活检标本,相应分为胆道闭锁(BA)组、胆总管囊肿(CCC)组及正常肝(NL)组.分别应用免疫组织化学法、逆转录-多聚酶链反应从蛋白和基因mRNA水平检测对比三组肝组织中MTA3、Snail1、E-cadherin表达情况.结果 在蛋白水平及mRNA水平BA组肝组织中Snail1呈高表达,MTA3、E-cadherin均呈低表达,MTA3和Snail1的表达呈负相关(r=-0.671,P<0.05;r=-0.862,P<0.05),Snail1和E-cadherin的表达呈负相关(r=-0.571,P<0.05;r=-0.715,P<0.05),MTA3和E-cadherin的表达呈正相关(r=0.671,P<0.05;r=0.752,P <0.05).结论 肝组织中MTA3减少,可能会降低对Snail1的抑制,进一步抑制E-cadherin的表达,促使胆管上皮EMT发生,导致BA的发生与发展.

목적 탐토담도폐쇄(biliary atresia,BA)환인간조직중전이상관기인3(MTA3)、자지전록인자Snail1、E-개점소(E-cadherin)삼자적표체、상관성급기재담도폐쇄발생、발전중적작용.방법 선취2012년2월지2013년9월하북의과대학제이의원소인외과수치적21례담도폐쇄、17례담총관낭종급10례간파렬환인적간조직활검표본,상응분위담도폐쇄(BA)조、담총관낭종(CCC)조급정상간(NL)조.분별응용면역조직화학법、역전록-다취매련반응종단백화기인mRNA수평검측대비삼조간조직중MTA3、Snail1、E-cadherin표체정황.결과 재단백수평급mRNA수평BA조간조직중Snail1정고표체,MTA3、E-cadherin균정저표체,MTA3화Snail1적표체정부상관(r=-0.671,P<0.05;r=-0.862,P<0.05),Snail1화E-cadherin적표체정부상관(r=-0.571,P<0.05;r=-0.715,P<0.05),MTA3화E-cadherin적표체정정상관(r=0.671,P<0.05;r=0.752,P <0.05).결론 간조직중MTA3감소,가능회강저대Snail1적억제,진일보억제E-cadherin적표체,촉사담관상피EMT발생,도치BA적발생여발전.
Objective To explore the expressions and correlations of MTA3,Snail1 and E-cadherin genes in the livers of Biliary Atresia (BA) and to analyze their roles in the development and progression of BA.Methods From February 2012 to September 2013,21 children with BA were enrolled into the experimental group.Meanwhile 17 patients with congenital choledochal cyst (CCC) and 10 patients with hepatic injuries (normal liver,NL) were enrolled into the control groups respectively.The expressions of MTA3,Snail1 and E-cadherin of the liver biopsies in the three groups were compared by immunohistochemistry.Also,Reverse transcription-polymerase chain reaction (RT-PCR) was applied to measure the mRNA expressions of MTA3,Snail1 and E-cadherin of the liver specimens in the three groups.Results In both mRNA and protein,a higher expression of Snail1 and a lower expression of MTA3 and E-cadherin were detected in the liver tissues of BA.Negative correlations existed between the MTA3 and Snail1 expressions (r =-0.671,P < 0.05;r =-0.862,P < 0.05),as well as between the Snail1 and E-cadherin expressions in the BA liver tissues (r =-0.571,P < 0.05;r =-0.715,P < 0.05).On the contrary,a positive correlation was shown between MTA3 and E-cadherin in the BA liver tissues (r =0.671,P < 0.05;r =0.752,P < 0.05).Conclusion Downregulation of MTA3 expression was related to EMT in the development and progression of BA by promoting Snail1 expression and inhibiting E-cadherin expression.
