目的:探讨甲状腺超声造影非均匀低增强模式对甲状腺癌的诊断价值。方法应用微泡造影剂(SonoVue)对本院2011年8月~2014年10月收治的147例甲状腺结节患者行超声造影检查,按照病理结果分为良性、恶性组,分析非均匀低增强模式在两组中的比例。结果147例甲状腺结节中,病理诊断结果:66例为恶性结节,81例为良性结节。恶性结节中62例超声造影表现为非均匀低增强,4例等增强;良性结节中5例超声造影表现为非均匀低增强,74例等增强,2例高增强。非均匀低增强超声造影增强模式对甲状腺癌诊断的准确度、灵敏度、特异度、阳性预测值、阴性预测值分别为93.8%、93.9%、93.8%、92.5%、95.0%。结论非均匀低增强超声造影增强模式对甲状腺癌的诊断价值很大。

목적:탐토갑상선초성조영비균균저증강모식대갑상선암적진단개치。방법응용미포조영제(SonoVue)대본원2011년8월~2014년10월수치적147례갑상선결절환자행초성조영검사,안조병리결과분위량성、악성조,분석비균균저증강모식재량조중적비례。결과147례갑상선결절중,병리진단결과:66례위악성결절,81례위량성결절。악성결절중62례초성조영표현위비균균저증강,4례등증강;량성결절중5례초성조영표현위비균균저증강,74례등증강,2례고증강。비균균저증강초성조영증강모식대갑상선암진단적준학도、령민도、특이도、양성예측치、음성예측치분별위93.8%、93.9%、93.8%、92.5%、95.0%。결론비균균저증강초성조영증강모식대갑상선암적진단개치흔대。
Objective To explore the diagnostic value of non uniform low enhanced model of thyroid ultrasound con-trast in thyroid carcinoma. Methods From August 2011 to October 2014,147 patients with thyroid nodule treated in our hospital were detected by ultrasound contrast using micro bubble contrast agent (SonoVue).Patients were divided into benign and malignant group according to pathological result.The proportion of non uniform low enhanced model in two groups. Results There were 66 malignant nodules and 81 benign nodules in 147 nodules according to pathological re-sult.Among the malignant nodules,ultrasound contrast performance of 62 nodules was non uniform low enhancement,and 4 nodules performed equal enhancement;among the benign nodules,5 nodules performed non uniform low enhancement, 74 cases performed equal enhancement,and 2 cases performed high enhancement.The diagnostic accuracy,sensitivity, specificity,positive predictive value and negative predictive value of non uniform low enhanced model for diagnosis of thyroid carcinoma was 93.8%,93.9%,93.8%,92.5% and 95.0% respectively. Conclusion Non uniform low enhanced ul-trasound contrast model has a great diagnostic value in thyroid carcinoma.
