目的：观察标准大骨瓣开颅与常规骨瓣开颅治疗重型颅脑损伤的临床疗效。方法选取2013年6月至2014年5月我院收治的70例重型颅脑损伤患者，随机分为两组，每组35例。对照组实施常规骨瓣开颅术，实验组实施大骨瓣开颅术，观察两组颅内压变化、治疗效果及并发症发生情况。结果实验组颅内压降压效果与治疗效果明显优于对照组（P＜0.05）；两组并发症发生率差异无统计学意义（P＞0.05）。结论大骨瓣开颅术治疗重型颅脑损伤患者效果更佳，减压效果更优，且具有较高的安全性。
목적：관찰표준대골판개로여상규골판개로치료중형로뇌손상적림상료효。방법선취2013년6월지2014년5월아원수치적70례중형로뇌손상환자，수궤분위량조，매조35례。대조조실시상규골판개로술，실험조실시대골판개로술，관찰량조로내압변화、치료효과급병발증발생정황。결과실험조로내압강압효과여치료효과명현우우대조조（P＜0.05）；량조병발증발생솔차이무통계학의의（P＞0.05）。결론대골판개로술치료중형로뇌손상환자효과경가，감압효과경우，차구유교고적안전성。
Objective To observe the standard craniotomy with conventional craniotomy treatment of severe traumatic brain injury clinical efficacy. Methods 70 cases of severe traumatic brain injury patients in our hospital from June 2013 to May 2014, were randomly divided into the two groups, 35 cases of each group. The control group were treated with conventional craniotomy, the experimental group was given the large trauma craniotomy. Changes in intracranial pressure, the incidence of treatment effects and complications were observed. Results Compared with the control group, the intracranial pressure antihypertensive effect and treatment effect in the experimental group were better, there was statistical signiifcance (P<0.05);complication rate between the two groups was no statistically signiifcant (P>0.05). Conclusion For patients with severe head injury, a large craniotomy better treatment, better decompression effect, and has a high safety.
