目的:检测糖尿病视网膜病变(DR)患者经曲安奈德治疗后,眼动脉、视网膜中央动脉收缩期血流速峰值(Peak Systolic Velocity; PSV)的变化,探讨曲安奈德对其血流动力学的影响。方法对13例糖尿病视网膜病变患者(13只眼)经玻璃体腔注射曲安奈德治疗前后,应用彩色多普勒超声成像技术对比研究眼动脉及视网膜中央动脉的收缩期血流速峰值(PSV)的变化。所有患者都随访3个月,分别于治疗后1周、1个月、3个月测量眼动脉及视网膜中央动脉的收缩期血流速峰值。结果所有患者都完成了治疗并随访3个月,视网膜中央动脉(CRA)的收缩期血流速峰值(PSV)在治疗前与治疗后1周,无明显差别(P>0.05)。在治疗前与治疗后1月,3月两时间点上测量,眼动脉(OA)及视网膜中央动脉(CRA)的收缩期血流速峰值(PSV)有显著差异(P<0.05)。结论曲安奈德(TA)可以通过抑制炎症反应,下调炎症因子,抑制新生血管形成,改善眼动脉和视网膜中央动脉的血管张力,从而改善动脉血流速峰值,对减少糖尿病视网膜病变的发展起到一定作用。

목적:검측당뇨병시망막병변(DR)환자경곡안내덕치료후,안동맥、시망막중앙동맥수축기혈류속봉치(Peak Systolic Velocity; PSV)적변화,탐토곡안내덕대기혈류동역학적영향。방법대13례당뇨병시망막병변환자(13지안)경파리체강주사곡안내덕치료전후,응용채색다보륵초성성상기술대비연구안동맥급시망막중앙동맥적수축기혈류속봉치(PSV)적변화。소유환자도수방3개월,분별우치료후1주、1개월、3개월측량안동맥급시망막중앙동맥적수축기혈류속봉치。결과소유환자도완성료치료병수방3개월,시망막중앙동맥(CRA)적수축기혈류속봉치(PSV)재치료전여치료후1주,무명현차별(P>0.05)。재치료전여치료후1월,3월량시간점상측량,안동맥(OA)급시망막중앙동맥(CRA)적수축기혈류속봉치(PSV)유현저차이(P<0.05)。결론곡안내덕(TA)가이통과억제염증반응,하조염증인자,억제신생혈관형성,개선안동맥화시망막중앙동맥적혈관장력,종이개선동맥혈류속봉치,대감소당뇨병시망막병변적발전기도일정작용。
Objective To check out the diabetic retinopathy (DR) patients’ changes of peak systolic velocity (PSV) of ophthalmic artery and central retinal artery after treated by triamcinolone acetonide and explore the influence of triamcinolone acetonide on hemodynamics. Methods Thirteen eyes of thirteen diabetic retinopathy patients treated by triamcinolone acetonide were studied for the changes of PSV of ophthalmic artery and central retinal artery by color Doppler ultrasound. All patients were followed up for 3 months. The PSV of ophthalmic artery and central retinal artery were measured at the one week, one month and three months after treatment. Results All patients were treated and followed up for 3 months. There was no significant difference in PSV of central retinal artery between pre-treatment and one week after the treatment (P>0.05). But there was significant difference in PSV between pre-treatment and one month and three months after the treatment(P<0.05). Conclusion Triamcinolone acetonide (TA) could inhibit neovascularization by suppressing inflammatory reaction and reducing inflammatory factor, thus improving the angiotasis of ophthalmic artery and central retinal artery, and peak systolic velocity as well. It is beneficial for inhibiting the progression of diabetic retinopathy.
