目的::评价多种肿瘤相关抗原(TAA)组合在肿瘤诊断中的价值。方法:应用 ELISA 法检测304份不同类型肿瘤(98例肺癌、69例食管癌、50例肝癌、46例结直肠癌、41例乳癌)患者和58份正常人血清中8种 TAA (Koc、p62、IMP1、cyclinE、p16、survivin、c-myc、p53)的自身抗体,用蛋白印迹技术验证其免疫活性,评价用 TAA 诊断肿瘤的真实性、可靠性和收益。结果:肿瘤患者血清中8种 TAA 阳性率普遍偏低,随着 TAA 的增多直至8种 TAA组合,对肿瘤的诊断灵敏度达到63.5%,特异度达86.2%,对不同类型肿瘤诊断的约登指数为0.423~0.522,阳性、阴性预测值为74.2%~88.7%、58.8%~75.8%,阳性、阴性似然比4.07~4.76、0.39~0.51,8种 TAA 诊断肿瘤的一致率和 Kappa 值分别为67.1%和0.51。结论:多种 TAA 组合对肿瘤具有较高的诊断价值。

Aim: To evaluate the diagnostic value of multiple tumor-associated antigens(TAAs) in different types of cancer. Methods: Eight TAAs including Koc, p62, IMP1, cyclinE, p16, survivin, c-myc and p53 in sera from 304 canc-er patients (98 cases of lung cancer,69 cases of esophageal cancer,50 cases of liver cancer,46 cases of colorectal cancer and 41 cases of breast cancer) and 58 normal individuals were detected by ELISA. Positive results of ELISA were con-firmed by Western blot. Results: The positive rate of the 8 TAAs in different cancer patients were generally low. With the successive addition of TAA to a final total of eight antigens, there was a stepwise increase of positive reactions reaching to a sensitivity of 63. 5% and a specificity of 86. 2% . In different cancer, the Youden′s index was 0. 423 - 0. 522, positive and negative predictive value was 74. 2% - 88. 7% and 58. 8% - 75. 8% , while positive and negative likelihood ratio was 4. 07 - 4. 76 and 0. 39 - 0. 51, which showed that the clinical diagnostic value of a assays of 8 TAAs was high. Agreement rate and Kappa value were 67. 1% and 0. 51, respectively. Conclusion: Multiple TAA arrays may have higher diagnostic value for tumor diagnosis.
