目的：：探讨超声乳化联合植入不同非球面人工晶体治疗白内障术后视觉及对比敏感度变化情况。方法：将72例96眼白内障患者分为A组、B组、C组,每组24例32眼,均给予超声乳化联合植入非球面人工晶体,A组采用AcrySof ReSTOR+3D (SN6AD1)多焦点非球面人工晶体,B组采用AcrySof ReSTOR+4D(SN6AD3)多焦点非球面人工晶体,C组采用单焦点球面人工晶体。比较3组患者术后1d、1周、1个月的裸眼和矫正视力,以及不同照明条件和空间频率下的对比敏感度。结果：A组术后1 d、1周、1个月的裸眼视力显著优于B组和C组(P<0.01),A组和B组患者裸眼近视力与裸眼远视力差异均无统计学意义(P>0.05),但均优于C组(P<0.05~P<0.01)；3组患者最佳矫正视力差异均无统计学意义(P>0.05)；非眩光(3 cd/m2)条件下,在3 c/d、6 c/d的低空间频率下A组和B组对比敏感度显著优于C组(P<0.01),在眩光(85 cd/m2)条件下,A组和B组对比敏感度均显著优于C组(P<0.01)。结论：超声乳化联合人工晶体植入术能够显著改善白内障患者的视觉质量,而与单焦点球面人工晶体相比,多焦点非球面人工晶体对患者对比敏感度的改善更佳,其中AcrySof ReSTOR+3D(SN6AD1)多焦点非球面人工晶体能够更显著地提高患者的中距离视力,值得临床推广应用。

Objective:To investigate the effects of phacoemulsification combined with implanting different aspheric intraocular lens on postoperative visual acuity and contrast sensitivity. Methods:Ninety-six cataract eyes(72 patients) were divided into group A,B and C. All cases were treated with phacoemulsification combined with implanting different aspheric intraocular lens. The AcrySof ReSTOR plus 3D(SN6AD1) aspheric multifocal intraocular lens,AcrySof ReSTOR plus 4D(SN6AD3) and monofocal intraocular lens were used in group A,B and C,respectively. The naked eye and corrected visual acuity,and contrast sensitivity in different lighting conditions and spatial frequency between 3 groups were compared at postoperative 1 day,1 week and 1 month. Results:The naked eye visual acuity in group A at postoperative 1 day,1 week and 1 month was significantly better than that in group B and group C(P<0. 01). The naked eye distance vision and near vision in group A and group B were better than that in group C,the differences of the naked eye distance vision and near vision between group A and group B were not statistically significant(P>0. 05). The contrast sensitivities in group A and group B under non-glare(3 cd/m2) and 3 c/d and 6 c/d low spatial frequency were better than that in group C(P<0. 01). The contrast sensitivities in group A and group B under glare(85 cd/m2) were better than that in group C(P<0. 01). Conclusions:The phacoemulsification combined with implanting intraocular lens can significantly improve the visual quality of cataract patients. Compared with the monofocal intraocular lens,the effect of aspheric multifocal intraocular lens on improving the contrast sensitivity is better. The AcrySof ReSTOR plus 3D(SN6AD1) aspheric multifocal intraocular lens can significantly improve the distance vision of patients,which is worthy of clinical application.
