目的：探析应用胎头娩出速度控制手法进行分娩在预防产妇会阴裂伤方面的临床价值。方法选择原阳县红十字医院2012年1月至2014年1月收治的90例初产妇进行研究，分为观察组和对照组各45例。观察组对胎儿娩出速度进行控制，对照组采取传统会阴保护方法，比较两组患者的新生儿体质量、新生儿评分、第二产程时间、住院时间以及会阴裂伤率。结果观察组的新生儿体质量和第二产程时间分别为（3.11±0.13）kg、（55.81±9.53）min，与对照组接近（P>0.05）；住院时间为（2.42±0.40）d，比对照组短（P<0.05），比较差异有统计学意义；观察组共12例为Ⅰ度裂伤，3例为Ⅱ度裂伤，无Ⅲ度裂伤，会阴裂伤率为33.33%，明显低于对照组的60.00%，比较差异具有统计学意义（ P<0.05）。结论胎头娩出速度控制手法在产妇分娩过程中的应用与传统会阴保护方法相比虽然不会对第二产程以及新生儿体质量造成影响，但是可以缩短产妇的住院时间并防止会阴裂伤问题，降低发生几率，应用效果满意，值得推广。

목적：탐석응용태두면출속도공제수법진행분면재예방산부회음렬상방면적림상개치。방법선택원양현홍십자의원2012년1월지2014년1월수치적90례초산부진행연구，분위관찰조화대조조각45례。관찰조대태인면출속도진행공제，대조조채취전통회음보호방법，비교량조환자적신생인체질량、신생인평분、제이산정시간、주원시간이급회음렬상솔。결과관찰조적신생인체질량화제이산정시간분별위（3.11±0.13）kg、（55.81±9.53）min，여대조조접근（P>0.05）；주원시간위（2.42±0.40）d，비대조조단（P<0.05），비교차이유통계학의의；관찰조공12례위Ⅰ도렬상，3례위Ⅱ도렬상，무Ⅲ도렬상，회음렬상솔위33.33%，명현저우대조조적60.00%，비교차이구유통계학의의（ P<0.05）。결론태두면출속도공제수법재산부분면과정중적응용여전통회음보호방법상비수연불회대제이산정이급신생인체질량조성영향，단시가이축단산부적주원시간병방지회음렬상문제，강저발생궤솔，응용효과만의，치득추엄。
Objective To analyze the clinical value of fetal head delivery time control technique on prevention of maternal perineal laceration. Methods Ninety primip-ara were selected from January 2012 to January 2014 as research objects,and they were divided into observation group and control group. The fetal childbirth speed was controlled in observation group,control group adopted the traditional perineal protection method. The neonatal birth weight score,the second labor time,length of hospital stay and the rate of perineal laceration in the two groups was compared. Results In observation group,birth weight and the second labor time respec-tively was(3.11 ±0.13)kg,(55.81 ±9.53)min,which has no significant difference with the control group(P>0.05);length of hospital stay[(2. 42 ± 0. 40)d]was shorter than that of control group,the difference was statistical significance ( P<0. 05);a total of 12 cases of observation group hadⅠdegree laceration,3 cases of Ⅱ degree laceration,no Ⅲ de-gree laceration,the perineal laceration rate was 33. 33%,which was significantly lower than that of the control group (60. 00%),there was significant difference(P<0. 05). Conclusions Fetal head delivery time control technique in the application of puerpera childbirth compared with the traditional perineal protection methods,it has no effect on the second labor and birth weight,but it can shorten the hospitalization time of maternal and prevent the perineal laceration and reduce risk with satisfying application effect,so it is worthy of promoting.
