目的 检测吸烟致COPD患者肺组织T辅助细胞17(Th17)分布及IL-17水平变化,探讨吸烟导致的COPD慢性炎症反应的可能机制.方法 收集2013年9月至2014年3月在我院因“孤立性肺结节”行手术切除肺叶患者的肺组织标本共56例,并将其分组:吸烟COPD组、吸烟肺功能正常组、不吸烟COPD组、不吸烟肺功能正常组.收集各组患者肺组织标本,HE染色法观察肺组织形态,实时荧光定量PCR方法检测Th17转录因子维甲酸受体相关孤儿受体t(RORγt)mRNA及IL-17 mRNA在肺组织的转录水平,ELISA法检测IL 17因子水平.结果 吸烟COPD组气道炎症病理评分明显增高,其RORγt、IL-17mRNA水平及IL-17均高于另外3组(P<0.05);吸烟肺功能正常组较非吸烟肺功能正常组气道炎症水及RORγt、IL 17mRNA及IL 17细胞因子水平增高.结论 吸烟相关COPD患者肺组织中存在Th17优势表达,IL-17表达水平与气道慢性炎症相关,Th17可能在吸烟所致COPD的发病机制中发挥重要作用.

Objective To investigate the Th17 cell distribution and expression of IL-17 in tobaccosmoke correlated COPD patients lung tissue.Methods Fifty-six cases of underwent solitary pulmonary nodules lobectomy from Peking University Shenzhen Hospital from September 2013 to March 2014 were recruited in this study.They were devided into smoked COPD group,smoked with normal lung function group,non smoked COPD group,non smoked with normal lung function group.All of lung tissue samples were collected,observed the lung tissue morphology by HE staining.The expression of Th17 specific transcription factor RORγt and IL-17 at mRNA level in lung tissue were determined by quantitative rea1-time PCR.The concentrations of IL-17 cytokines in lung samples werc measured by enzyme-linked immunosorbent assay (ELISA).Results Compared with the other three groups,smoked COPD group had dramatically increased airway inflammatory pathologic score,and showed higher level of the expression of RORγt,IL-17mRNA and IL 17 cytokine concentration.In smoked with normal lung function group,RORγt,IL-17 were up-regulated at transcriptional leve1,IL-17 cytokine concentration also increased than the other two non smoked group.Conclusions Th17 cell preferentially expressed in COPD patients lung tissue,the effective cytokine level of IL-17 was correlated with severity of chronic inflammation.Th17 maybe play an important role in the pathogenesis in chronic obstructive pulmonary disease.
