目的:探讨重症结核病患者细胞免疫和体液免疫的变化情况。方法将2713例结核病患者根据病情分成普通结核组(2557例)及重症结核组(156例),比较并分析两组患者外周血CD3+T细胞、CD4+T细胞、CD8+T细胞、自然杀伤(NK)细胞、B细胞、免疫球蛋白(Ig)A、IgG、IgM、补体C3及补体C4的水平。结果重症结核组患者外周血CD3+T细胞、CD4+T细胞、NK细胞及B细胞较普通结核组患者均明显下降(P均<0.05),而CD8+T细胞两组比较,差异无统计学意义(P>0.05)。同时,重症结核组患者外周血IgA、IgG、IgM、补体C3及补体C4均较普通结核组患者均显著下降(P均<0.05)。结论重症结核病患者的细胞免疫和体液免疫均比普通结核病患者受到了更大程度的损害。

목적:탐토중증결핵병환자세포면역화체액면역적변화정황。방법장2713례결핵병환자근거병정분성보통결핵조(2557례)급중증결핵조(156례),비교병분석량조환자외주혈CD3+T세포、CD4+T세포、CD8+T세포、자연살상(NK)세포、B세포、면역구단백(Ig)A、IgG、IgM、보체C3급보체C4적수평。결과중증결핵조환자외주혈CD3+T세포、CD4+T세포、NK세포급B세포교보통결핵조환자균명현하강(P균<0.05),이CD8+T세포량조비교,차이무통계학의의(P>0.05)。동시,중증결핵조환자외주혈IgA、IgG、IgM、보체C3급보체C4균교보통결핵조환자균현저하강(P균<0.05)。결론중증결핵병환자적세포면역화체액면역균비보통결핵병환자수도료경대정도적손해。
Objective To explore the changes and clinical significance of cellular immunity and humoral immunity in patients with severe tuberculosis (TB). Methods A total of 2 713 TB patients were divided into the ordinary TB group (2 557 cases) and severe TB group (156 cases). Peripheral blood lymphocyte CD3 +T cell, CD4+T cell, CD8+T cell, natural killer cell, B cell, the peripheral plasma level of immunoglobulin (Ig) A, IgG, IgM, complement C3 and C4 were analyzed and compared. Results Compared with the ordinary TB group, the ratio of CD3+T cell, CD4+T cell natural killer cell and B cell in the severe TB group were all decreased (all P<0.05), but the ratio of CD8+T cell showed no significant difference between the two group (P>0.05). Meanwhile, the plasma level of IgA, IgG, IgM and complement C3 and C4 in the severe TB group were all lower than those in the ordinary TB group (all P<0.05). Conclusion The cellular immunity and humoral immunity in the severe TB patients suffered more damage as compare with the ordinary TB patients.
