目的：通过某郊区二级医院近2年出院患者的病案首页数据统计分析，了解超长住院患者对平均住院日的影响。方法将出院患者住院日的第99百分位数及以上定义为超长住院，调取2012年1月到2013年12月出院患者的病案首页信息，计算超长住院日。对超长住院患者进行统计分析，了解其对平均住院日的影响及分布情况。结果139例超长住院患者中，付费方式前2位的是医疗保险和新农合，分别占46.8%和25.2%；出院科室第一位的是骨科，占46.8%；出院主要诊断第一位的是腰椎间盘突出，占20.9%。结论超长住院患者对平均住院日影响巨大，探索超长住院患者的有效管理机制，对缩短平均住院日，合理利用医疗资源有重要意义。

Objective To evaluate the effect of patients with long hospital stay on the average hospital stay by analyzing the first page of medical records of patients discharged in recent two years in a suburban secondary hospital. Methods The author defines discharged patients of 99% or more for long hospital stay. After retrieval of the first page of medical records in patients discharged from January 2012 and December 2013 and identification of patients with long hospital stay, we performed statistical analysis on these patients and evaluated their effect on the average hospital stay and their distribution.Results In the identified 139 patients with long hospital stay, the two predominant payment types were through medical insurance and new rural cooperative medical system, with proportions of 46.8% and 25.2%, respectively. The most of patients(46.8%) with long hospital stay were discharged from the department of orthopedics, with the predominant diagnosis of lumbar disc protrusion(20.9%). Conclusions Patients with long hospital stay have a huge impact on the average hospital stay.Continualreform to reduce patients with long hospital stay will have a crucial effect on shortening average hospital stay and appropriate usage of medical resources.
