目的:了解天津市全科医生的执业满意度状况,并探讨影响其满意度的因素,提出改善全科医生执业满意度的建议。方法以天津市在社区卫生服务机构全科岗位工作的所有医生为总体,采用整群分层随机抽样的方法,抽取400名全科医生为调查对象。于2014年4月17—27日,采用“全科医务人员执业满意度调查问卷”进行问卷调查。调查问卷的主要内容包括:全科医生的基本情况,对人员晋升、培训、工作压力、收入水平、社会认可的满意度,总的执业满意度。结果发放问卷400份,回收有效问卷373份,有效回收率为93.2%。373名全科医生中,对人员晋升表示满意、一般、不满意的分别为92人(占24.7%)、150人(占40.2%)、131人(占35.1%);对培训表示满意、一般、不满意的分别为148人(占39.7%)、96人(占25.7%)、129人(占34.6%);对工作压力表示满意、一般、不满意的分别为5人(占1.4%)、12人(占3.2%)、356人(占95.4%);对收入水平表示满意、一般、不满意的分别为104人(占27.9%)、64人(占17.1%)、205人(占55.0%);对社会认可表示满意、一般、不满意的分别为200人(占53.6%)、127人(占34.0%)、46人(占12.4%)。对总体执业表示满意、不满意的分别为97人(占26.0%)、276人(占74.0%)。 Logistic回归分析显示,全科医生的年龄及对晋升、工作压力、收入水平、社会认可的满意度对总体执业满意度的影响有统计学意义( P<0.05)。结论天津市全科医生的执业满意度不高,影响执业满意度的主要因素包括全科医生的年龄及对晋升、工作压力、收入水平、社会认可的满意度。应通过开展全科医生结构性调整,建立专业化职称评定体系,完善全科培训管理体系,合理制定绩效工资分配制度以及强化全科医生自我认同感和社会认可度,提高全科医生的执业满意度。

Objective To understand the satisfaction degree of general practitioners in Tianjin with practice , explore its influencing factors and make suggestions for the improvement of satisfaction degree of general practitioners with their practice.Methods Of all doctors involved in general practice in community health service settings in Tianjin , we selected 400 general practitioners as subjects using cluster stratified random sampling method .From April 17 to 27, 2014, we conducted questionnaire survey using "the questionnaire of satisfaction degree of general practitioners with their practice ".The questionnaire mainly included the basic information , personnel promotion , training , and pressure from work , income level , satisfaction degree with social recognition degree and overall satisfaction degree with practice .Results We distributed 400 questionnaires and recovered 373 questionnaires , with an effective returning rate of 93.2%.Among 373 subjects , the numbers of subjects who were satisfied , barely satisfied and unsatisfied with personnel promotion were 92 (24.7%), 150 (40.2%) and 131 (35.1%) respectively; the numbers of subjects who were satisfied , barely satisfied and unsatisfied with training were 148 (39.7%), 96 (25.7%) and 129 (34.6%) respectively; the numbers of subjects who were satisfied , barely satisfied and unsatisfied with pressure from work were 5 (1.4%), 12 (3.2%) and 356 (95.4%) respectively; the numbers of subjects who were satisfied , barely satisfied and unsatisfied with income level were 104 ( 27.9%) , 64 ( 17.1%) and 205 ( 55.0%) respectively; the numbers of subjects who were satisfied , barely satisfied and unsatisfied with social recognition were 200 (53.6%), 127 (34.0%) and 46 (12.4%) respectively; the numbers of subjects who were satisfied and unsatisfied with overall practice were 97 (26.0%) and 276 (74.0%) .The logistic regression analysis showed that age and satisfaction degree with personnel promotion , pressure from work , income level and social recognition had significant influence ( P<0.05 ) on the satisfaction degree with overall practice .Gonclusion The overall satisfaction degree of general practitioners in Tianjin with practice is low, the major influencing factors for the satisfaction degree with practice include age and the satisfaction degree with promotion , pressure from work , income level and social recognition .Structural adjustment of general practice personnel should be carried out, the professionalized evaluation system of professional title should be established , the training and management system of general practice should be promoted , performance related pay system should be formulated rationally , and self -recognition and social recognition degree of general practitioners should be strengthen , in order to improve the satisfaction degree of general practitioners with practice.
