目的:观察骨转换标志物对慢性肾脏病患者并发骨营养不良的早期诊断价值。方法分别检测78例慢性肾脏病患者和46例健康成人血清中骨转换标志物总Ⅰ型前胶原氨基端延长肽(TPINP)、Ⅰ型胶原羧基端肽β特殊序列(β-CTx)、完整甲状旁腺素(iPTH)及血清1,25-二羟维生素D3[1,25-(OH)2D3]水平,并比较其差异性。结果慢性肾脏病并发骨营养不良患者血清TPINP、β-CTx及iPTH水平均显著高于正常对照组(P均<0.01),而血清1,25-(OH)2D3水平则显著低于对照组(P<0.01)。结论对慢性肾脏病患者定期进行骨转换标志物监测,有助于肾性骨营养不良的早期诊断。

목적:관찰골전환표지물대만성신장병환자병발골영양불량적조기진단개치。방법분별검측78례만성신장병환자화46례건강성인혈청중골전환표지물총Ⅰ형전효원안기단연장태(TPINP)、Ⅰ형효원최기단태β특수서렬(β-CTx)、완정갑상방선소(iPTH)급혈청1,25-이간유생소D3[1,25-(OH)2D3]수평,병비교기차이성。결과만성신장병병발골영양불량환자혈청TPINP、β-CTx급iPTH수평균현저고우정상대조조(P균<0.01),이혈청1,25-(OH)2D3수평칙현저저우대조조(P<0.01)。결론대만성신장병환자정기진행골전환표지물감측,유조우신성골영양불량적조기진단。
ObjectiveTo investigate the value of application of biochemical marker of bone turnover in the early diagnosis of renal osteodystrophy in patients with chronic kidney disease.MethodsA total cases of 78 patients with chronic kidney disease and 46 healthy peoples were included in this study.The serum levels of total procollagen typeⅠamino-terminal propeptide(TPINP),β-isomerized carboxyterminal propeptide (β-CTx),intact parathyroid hormone(iPTH),and serum 1,25-dihydroxyvitamin D3[1,25-(OH)2D3] were measured,then the difference among them was be compared.ResultsThe serum levels of TPINP,β-CTx and iPTH in the patients with CKD group was significantly higher then the healthy subjects(均P<0.01),the serum 1,25-(OH)2D3 level in the CKD group was significantly decreased compared with that in the control(P<0.01).ConclusionThe application of periodic monitor the bone turnover markers were useful for the early diagnosis of renal osteodystrophy in patients with chronic kidney disease.
