目的:观察恶性腹水中腹腔化疗前后癌胚抗原信使核糖核酸(Carcinoembryonic antigen message RNA,CEAmRNA)和糖链抗原19-9(CA19-9)的变化及其意义。方法恶性腹水标本化疗前后各56份,采用逆转录套式聚合酶链反应(RT-NP-PCR)检测CEAmRNA,电化学发光法检测CA19-9。结果 CEAmRNA检出率由化疗前98.2%(55/56)降至化疗后85.7%(48/56),P<0.05(x2=5.920);CA19-9由化疗前(69.4±11.5)μ/mL降至化疗后(52.1±9.3)μ/mL,P<0.01(t=8.550)。结论恶性腹水标本可以检测到较高水平的CEAmRNA和CA19-9阳性结果,提示二者在恶性腹水时被高度活性表达,当腹腔灌注化疗后,二者也显著降低,其测定值可作为疗效评价判别的参考。

Objective:To observe the effects of malignant ascites intraperitoneal chemotherapy carcinoembryonic antigen messenger ribonucleic acid (carcinoembryonic antigen message RNA, CEAmRNA) and carbohydrate antigen 19-9 (CA19-9) changes and its significance.Methods: 56 patients with malignant ascites were detected by reverse transcription nested polymerase chain reaction (RT-NP-PCR) and CEAmRNA, respectively, and CA19-9 was detected by using reverse transcription nested polymerase chain reaction Results: CEAmRNA detection rate by chemotherapy before 98.2% (55 / 56) fell to chemotherapy after 85.7% (48 / 56),P< 0.05 (x2=5.920); CA19-9 by before chemotherapy (69.4 + 11.5) U / ml decreased after chemotherapy (52.1 + 9.3) U / ml,P < 0.01 (t=8.550. Conclusion:Malignant ascites specimens can be detected higher levels of CEA and CA19-9 positive results, suggesting that both in malignant ascites was highly active expression, when intraperitoneal chemotherapy, both significantly decreased, the determination as a discriminant in efficacy evaluation of reference value.
