目的:探讨自身免疫性溶血性贫血(AIHA)患者血清含类同种自身抗体的比率、抗体相对特异性及对输血对策的影响。方法利用谱细胞鉴定AIHA患者血清中和红细胞上抗体,进一步选择具有合适抗原的细胞,通过吸收放散试验区分同种抗体和类同种自身抗体,同时测定血清中检出自身抗体的患者红细胞Rh血型。结果 AIHA患者血清类同种自身抗体的检出率为19.7%(14/71),均是抗Rh血型系统抗原。结论 AIHA患者类同种自身抗体相当常见,多为抗Rh血型系统抗原,对血清只含有类同种自身抗体的AIHA患者,可输注“最少不相容”红细胞,而不一定要输注抗原阴性红细胞。

목적:탐토자신면역성용혈성빈혈(AIHA)환자혈청함류동충자신항체적비솔、항체상대특이성급대수혈대책적영향。방법이용보세포감정AIHA환자혈청중화홍세포상항체,진일보선택구유합괄항원적세포,통과흡수방산시험구분동충항체화류동충자신항체,동시측정혈청중검출자신항체적환자홍세포Rh혈형。결과 AIHA환자혈청류동충자신항체적검출솔위19.7%(14/71),균시항Rh혈형계통항원。결론 AIHA환자류동충자신항체상당상견,다위항Rh혈형계통항원,대혈청지함유류동충자신항체적AIHA환자,가수주“최소불상용”홍세포,이불일정요수주항원음성홍세포。
Astract: Objective To investigate on the frequency of autoantibodies with mimicking specificity and antibody specificity, and the effects on blood transfusion countermeasures in patients with autoimmune hemolytic anemia (AIHA). Methods Identification of red cell antibodies was performed in sera of AIHA patients and antibody specificity were determined by using spectrum cells. The alloantibodies and the autoantibodies were identified with mimicking specificity by absorb radiation test. And the Red blood Rh blood type were detected. Results Autoantibodies with mimicking specificity were 19.7% (14/71) of the patients and all of them were directed against Rh system antigens. Conclusions Autoantibodies with mimicking specificity in AIHA patients are relatively frequent, and the transfusion of antigen-negative RBCs might not be mandatory when the patients have only autoantibodies with mimicking specificity, but"the least incompatible red blood cells"might be accepted.
