目的:探讨2型糖尿病患者随着血清炎症因子C反应蛋白(CRP)变化,其血清中的细胞因子(TNF-α、IL-10及IL-1β)及外周血单核细胞中P2X7受体的表达变化。方法通过免疫比浊法检测CRP水平,将2型糖尿病分为CRP升高2型糖尿病及CRP正常2型糖尿病患者;酶联免疫法(ELISA)检测20例正常对照组、18例CRP升高2型糖尿病和20例CRP正常2型糖尿病患者的血清细胞因子(TNF-α、IL-10及IL-1β)水平;RT-PCR法检测外周血单核细胞P2X7受体的mRNA表达水平(以β-actin为内参)。结果2型糖尿病组患者P2X7受体基因的表达水平显著高于正常对照组,而CRP正常2型糖尿病组及CRP升高2型糖尿病组之间无显著性差异;2型糖尿病患者的TNF-α、IL-1β的水平明显高于正常对照组,同时TNF-α及IL-1β的水平在CRP升高2型糖尿病组也明显高于CRP正常2型糖尿病组。 CRP升高2型糖尿病组的IL-10水平显著低于其他2组。结论单核细胞表达的P2X7受体基因及血清中的TNF-α、IL-10、IL-1β可能与2型糖尿病存在一定的关联性,它们在2型糖尿病发生及进展过程中可能具有重要的调控作用。

목적:탐토2형당뇨병환자수착혈청염증인자C반응단백(CRP)변화,기혈청중적세포인자(TNF-α、IL-10급IL-1β)급외주혈단핵세포중P2X7수체적표체변화。방법통과면역비탁법검측CRP수평,장2형당뇨병분위CRP승고2형당뇨병급CRP정상2형당뇨병환자;매련면역법(ELISA)검측20례정상대조조、18례CRP승고2형당뇨병화20례CRP정상2형당뇨병환자적혈청세포인자(TNF-α、IL-10급IL-1β)수평;RT-PCR법검측외주혈단핵세포P2X7수체적mRNA표체수평(이β-actin위내삼)。결과2형당뇨병조환자P2X7수체기인적표체수평현저고우정상대조조,이CRP정상2형당뇨병조급CRP승고2형당뇨병조지간무현저성차이;2형당뇨병환자적TNF-α、IL-1β적수평명현고우정상대조조,동시TNF-α급IL-1β적수평재CRP승고2형당뇨병조야명현고우CRP정상2형당뇨병조。 CRP승고2형당뇨병조적IL-10수평현저저우기타2조。결론단핵세포표체적P2X7수체기인급혈청중적TNF-α、IL-10、IL-1β가능여2형당뇨병존재일정적관련성,타문재2형당뇨병발생급진전과정중가능구유중요적조공작용。
Objective To investigate the changes of serum cytokines (TNF-α,IL-10 and IL-1β) and the P2X7 receptor expres-sion in peripheral blood monocytes from the type 2 diabetes mellitus patients with serum C reactive protein (CRP) changes. Methods According to the level of CRP detected by immune turbidity method, type 2 diabetes mellitus were divided into two groups:increased CRP type 2 diabetes mellitus and normal CRP type 2 diabetes mellitus. The cytokines (TNF- α,IL-10 and IL-1β)were assayed by enzyme linked immunosorbent assay (ELISA) including 20 people in normal control group, 18 patients with increased CRP type 2 diabetes mellitus and 20 patients with normal CRP type 2 diabetes mellitus. Meanwhile the P2X7 expression in peripheral blood monocytes was detected by RT-PCR (β-actin as internal reference). Results The expression level of P2X7 in patients with type 2 diabetes group was significantly higher than that in the normal control group, while there was no significant difference between nor-mal CRP type 2 diabetes mellitus and increased CRP type 2 diabetes mellitus. The levels of serum cytokines (TNF-α, IL-1β) in type 2 diabetes mellitus patients were significantly higher than those in normal control group as well, while TNF-αand IL-1βlevel in in-creased CRP type 2 diabetes group was significantly higher than those in normal CRP type 2 diabetes mellitus. In increased CRP type 2 diabetes mellitus group, IL-10 level was significantly decreased compared with the other two groups. Conclusion The levels of serum cytokines (TNF-α, IL-10, IL-1β) and P2X7 expression in monocytes may exist some correlation with type 2 diabetes mel-litus. There are likely to be a relevant mechanism in the occurrence and progress of T2DM patients.
