目的:探讨64层颅颈联合CTA诊断颅颈动脉狭窄的临床应用价值。方法:收集2012年9月到2014年9月在我院治疗的颅颈动脉狭窄患者75例,均行颅颈联合CTA及常规DSA造影。以DSA为金标准,评价颅颈联合CTA诊断颅颈动脉狭窄的敏感度和特异度;同时评价CTA与DSA在动脉狭窄程度评估上的一致性。结果:在动脉狭窄的检出率上CTA与DSA差异无统计学意义(P>0.05),颅颈联合CTA诊断颅颈动脉狭窄的敏感度、特异度分别为88.9%、99.1%;在评价颅颈动脉狭窄程度上, CTA与DSA的一致性较好,K=0.843。结论:64层颅颈联合CTA是可靠的血管成像方法,能较好地发现颅颈动脉狭窄,在评价颅颈动脉狭窄程度上与DSA有较好的一致性。

목적:탐토64층로경연합CTA진단로경동맥협착적림상응용개치。방법:수집2012년9월도2014년9월재아원치료적로경동맥협착환자75례,균행로경연합CTA급상규DSA조영。이DSA위금표준,평개로경연합CTA진단로경동맥협착적민감도화특이도;동시평개CTA여DSA재동맥협착정도평고상적일치성。결과:재동맥협착적검출솔상CTA여DSA차이무통계학의의(P>0.05),로경연합CTA진단로경동맥협착적민감도、특이도분별위88.9%、99.1%;재평개로경동맥협착정도상, CTA여DSA적일치성교호,K=0.843。결론:64층로경연합CTA시가고적혈관성상방법,능교호지발현로경동맥협착,재평개로경동맥협착정도상여DSA유교호적일치성。
Objective:To evaluate the application value of 64-slice spiral CT cranial-neck joint CTA in the diagnosis of cra-nial-neck artery stenosis. Methods:75 cases underwent the cranial-neck joint CTA and conventional DSA were collected. DSA image remains the gold standard. The sensitivity and specificity of cranial-neck joint CTA in diagnosis of cranial-neck artery stenosis were evaluated,the correlation of CTA with DSA were aralyzed. Results:In diagnosis of cranial-neck artery stenosis,the diagnosis rate had no difference between CTA and DSA (P>0.05),and the sensitivity and specificity of CTA were 88.9% and 99.1%,respectively. CTA was significantly correlated with DSA in evaluating the stenosis grade. The Kappa value was 0.843. Conclusions:The cranial-neck joint CTA is a reliable vascular imaging method,and it is significantly correlated with DSA in diagnosing cranial-neck artery stenosis.
