目的:对比常规颈椎过伸过屈位DR片及颈椎屈伸运动平板透视2种检查方法在诊断退变性下颈椎不稳中的价值,提高退变性下颈椎不稳的诊断水平。方法:48例疑诊为退变性下颈椎不稳患者分别行常规颈椎过伸过屈位摄片及颈椎屈伸运动平板透视。由同一名诊断医师盲法读片并记录椎体的水平位移及角位移数据,根据诊断标准判断是否存在颈椎不稳,对2种检查方法获得的计数及计量数据分别行配对χ2检验及t检验。结果:颈椎屈伸运动平板透视诊断阳性率为47.92%(23/48),较常规颈椎DR阳性率29.17%(14/48)高,2种检查方法诊断退变性下颈椎不稳的差异有统计学意义(P<0.05)。结论:颈椎屈伸运动平板透视较常规颈椎DR能够反映更多的颈椎运动信息,是诊断退变性下颈椎不稳的一种新方法。

Objective:To contrast the value of digital radiography (DR) and digital flat fluoroscophy in diagnosing degenerative instability of lower cervical vertebrae,in order to improve the diagnosis level. Methods:48 patients which were suspected of de-generative instability of lower cervical vertebrae were reviewed. All patients underwent DR and digital flat fluoroscophy. The vertebral horizontal displacement (HD) and angular displacement (AD) were measured and recorded by one doctor. The pairedχ2 test and t-test were used in this study. Results:The positive rate of digital flat fluoroscophy (47.91%) was higher than that of DR (29.16%),the difference was statistically significant (P<0.05). Conclusions:Digital flat fluoroscophy can show more in-formation about the movement of cervical vertebrae than DR. It is a new method in diagnosing degenerative instability of the lower cervical spine.
