目的:探讨MSCT模拟三维立体定位技术在经皮肺穿刺活检术导航中的临床应用价值。方法:选择112例胸部局限性肿块患者,随机分为实验组和对照组各56例,实验组采用MSCT三维立体定位与三维导航仪穿刺,对照组采用常规横断面内CT引导穿刺活检,比较2组一次性穿刺成功率、并发症发生率和活检诊断正确率。结果:一次性穿刺成功率实验组73.21%(41/56),对照组41.07%(23/56),2组比较差异有统计学意义(χ2=11.812,P<0.05);并发症发生率实验组16.07%(9/56),对照组33.93%(19/56),2组比较差异有统计学意义(χ2=4.762,P<0.05);病理确诊率实验组94.64%(53/56),对照组82.14%(46/56),2组比较差异有统计学意义(χ2=4.264,P<0.05)。结论:MSCT模拟三维立体定位,实现了术前穿刺路径的计算机模拟与三维定位,穿刺准确度与确诊率显著提高,并发症明显减少,为术中三维数据导航提供了可靠依据,具有较高的临床应用价值。

Objective:To explore the application value of MSCT simulated three-dimensional positioning in lung percutaneous needle biopsy. Methods:112 cases with lung local masses were randomly divided into experimental group and control group e-qually. The cases in experimental group underwent MSCT 3D positioning and 3D puncture biopsy instrument,while the cases in control group underwent conventional cross-sectional CT guided biopsy. The one-time success rate of biopsy puncture,complica-tion incidence rate and diagnostic accuracy were compared between the two groups. Results:The one-time success rate of biop-sy puncture of the experimental group and the control group was 73.21% (41/56) and 41.07% (23/56),respectively,there was a significant difference (χ2=11.812,P<0.05). The complication incidence rate was 16.07% (9/56) and 33.94% (19/56),respectively, there was a significant difference (χ2=4.762,P<0.05). The biopsy diagnostic accuracy was 96.43% (53/56) and 82.14% (46/56), respectively,there was a significant difference (χ2=4.264,P<0.05). Conclusions:MSCT simulated 3D positioning realizes the com-puter simulation and 3D positioning of preoperative puncture path,and increases the puncture and diagnosis accuracy,decreases the complication incidence rate,and provides a reliable basis for 3D data in the navigation. It has an important clinical value.
