目的：调查医嘱中存在的不合理用药问题，指导临床合理用药。方法抽取2013年1月至2013年12月出院病历600份，对其中不合理用药医嘱进行统计分类。参考药品说明书、《临床用药须知》、《新编药物学》、《临床静脉用药调配与使用指南》及其他文献等，对每一类不合理用药医嘱进行分析。结果不合理用药医嘱317条，占用药医嘱总条目数(22200)的1.43%。主要包括抗菌药物使用不规范、适应证不适宜、用法与用量不适宜、超说明书用药、溶媒选择不合理、联合用药不适宜等。结论医院应重视医嘱点评工作，提高合理用药水平，保障患者用药安全。
목적：조사의촉중존재적불합리용약문제，지도림상합리용약。방법추취2013년1월지2013년12월출원병력600빈，대기중불합리용약의촉진행통계분류。삼고약품설명서、《림상용약수지》、《신편약물학》、《림상정맥용약조배여사용지남》급기타문헌등，대매일류불합리용약의촉진행분석。결과불합리용약의촉317조，점용약의촉총조목수(22200)적1.43%。주요포괄항균약물사용불규범、괄응증불괄의、용법여용량불괄의、초설명서용약、용매선택불합리、연합용약불괄의등。결론의원응중시의촉점평공작，제고합리용약수평，보장환자용약안전。
Objective To investigate the problems of irrational use of drugs in medical orders in order to direct the rational use of drugs. Methods Six hundred medical records of discharged inpatient were selected in our hospital from January to December 2013 and statistically classiifed on the irrational medication orders. and the reasons of each category of them were analyzed according to the description of the medicines, "Clinical Guide", "New Pharmacology","Clinical Intravenous Drug Deployment and User Guide"and other literatures. Results There are 317 unreasonable doctor’s orders with failure rate of 1.43%(the total number is 22200),which mainly included antimicrobial drug use is not standard, indications are not appropriate, irrational usage and dosage of drug, drug use beyond the scope of package inserts, improper solvent selection, unreasonable combined use, etc. Conclusion Hospital should pay great attention to the comment work on medication orders to increase the level of rational drug use and ensure the medication safety of patients.
